The trust in public health institutions in Croatia has been declining for the third consecutive year, as indicated by the results of Nulti kvadrant, research ordered by Val grupa, organizer of the HealthComm Forum, at the end of 2023. Currently, only 29 percent of citizens trust public health institutions and only every fourth citizen is satisfied with public health services. This negative trend highlights the necessity of redefining the social contract in healthcare, which should be based on active listening and incorporating patients' interests into the decision-making system, focusing on and monitoring treatment outcomes, investing in new technology, making the latest scientific advancements available to patients, investing in the continuous education of all healthcare professionals, fostering continuous collaboration among all stakeholders, and ensuring timely and transparent communication. In 2015, at age 22, I began my career as a health tech entrepreneur by living in 3 nursing homes for a year, developing a product to enhance residents’ quality of life. Today, I am dedicated to ensuring individuals prevent chronic disease and prolong their healthspans for as long as possible. The province of Ontario in Canada has a population of 14.5 million people and is comparable to some EU Member States in size and healthcare delivery model. Like Member States, Ontario is facing challenges related to the financial and workforce burdens that threaten the sustainability of its healthcare system and is currently undergoing a transformation to modernize its systems to support patient journeys, by creating a province-wide health data fabric. Smile’s CTO, James Agnew, has been involved with the HL7 FHIR standard since 2011 and currently sits on the FHIR "Core Team" with editorial responsibilities for implementer support. James will delve into Ontario's roadmap for the health data fabric, as well as Canada's contributions to the International Patient Summary (IPS) specification. The talk will also suggest how Ontario's experience can inform and enrich the European Health Data Space (EHDS) evolution, particularly in the context of developing standardized, accessible, and secure health data exchange. Attendees will gain valuable insights into international best practices in liberating and leveraging health data in order to transform how healthcare services can be delivered in Europe and beyond.
