Who trains PharmD students during their internship period? Just as MBBS, MD, and BDS students undergo training under qualified senior physicians and dentists respectively, PharmD students should be able to learn practical skills under qualified clinical pharmacists in the hospital setting. There are more than 300 pharmacy colleges in India accredited with the PharmD program, but among those institutes, how many colleges are able to offer good facilities for their students during internship? Good teachers who teach textbooks well are just not enough when it comes to programs such as PharmD, where practical skills carries equal weight. PharmD interns should not face a situation where they are left in a hospital setting and have to figure out everything by themselves. They need to have supervision of clinical pharmacists during their training period. It has been almost 16 years since PharmD has been introduced in India and with the fees of PharmD programs in private colleges reaching a record high in our country, this is surely a thought worth considering.

🎉 successful completion 🎉 ✅ Here are valuable insights I acquired during my internship: 1. Throughout my internship, I gained a deep understanding of the intricate nature of IV admixture processes within hospitals. The complexity of these processes necessitates significant investments and high costs in every aspect, from medication reconstitution to the final administration to patients. 2. The administration of IV medications to critically ill patients, especially pediatric and neonatal cases, necessitates the presence of a dedicated IV mixing unit in every hospital. In this context, hospital pharmacists play a pivotal role as the primary qualified professionals. 3. Building such a unit requires a multitude of resources, starting with the construction of an IV unit room and the procurement of necessary materials and equipment, including an air filter system. Additionally, the Drug Information Center (DIC) proves to be a valuable resource, providing guidance to pharmacists throughout the process. 4. A crucial resource in the entire IV admixture process is the hospital information system, which enables comprehensive tracking of the entire workflow. This system facilitates the management of supplies from the main storage, review of medication protocols (especially for chemotherapy), dispensing, reconstitution and mixing, verification by clinical pharmacists, and the final delivery of medication bags or bottles to patient wards, ensuring seamless handover to the nursing staff. Lastly, I would like to extend my special thanks to my pharmacy team at CHG NBH and the dedicated IV unit team at 57357. 🫡

I started my internship at one of the largest & busiest hospitals in the country. This post is a culmination of my thoughts & reality-checks that hit me over the past week: 1. Knowledge is King: I entered my internship after clearing my USMLE STEP 1 & STEP 2 with great scores. This gave me a strong solid foundation on clinical subjects that helped me understand what is going in an inpatient setting. When you have knowledge, you easily understand why an investigation was ordered and why so-and-so medicine was prescribed or stopped for a patient. Your eyes can only recognise what the mind knows! 2. Decision Making is Key: The hardest part about medicine is not always diagnosing or prescribing medicines. It is decision making! When it comes to acute settings, a doctor’s presence of mind and courage to take a decision regarding management often draw the thin line between life and death. 3. Everyone is at distress: Everyone in a tertiary care setting is overworked or in distress. The junior doctors, house surgeons, residents, nurses, attenders and cleaning staff are all overworked. The patients & bystanders are at distress. It is too hard to blame a single entity for the failure in patient care when it happens. TRAINING OF DOCTORS, DENTISTS, PHARMACISTS,NURSES AND CLINICIANS. The training of the above cadre of professionals were generally based on the British system and later blended with the US system. They would undergo a period of academic and professional instruction and finally internship before registration.I am particularly curious about the matter of internship.The tradition for a long time has been that internship begins after the students' graduation from their university work. It's only after they are signed off from the internship that the various regulatory bodies can recognise them and license them to practice their professions. Interns as far as l can remember were paid some sort of salary and then moved to a higher job group on completion of internship. This was practised smoothly because all the interns were being absorbed by the ministry and later employed. At the moment the rate of production of medics has surpassed the capacity of the government to absorb all of them either as interns or qualified officers. This is causing a delay in terms of when and how long they may take before they are licensed to practice. This is despite the long periods they undergo during their studies and also not downplaying the acute shortage of the same medics in the public service. What then should change for internship problems to be solved? The universities and other relevant training institutions should rework their programmes where internship is an inbuilt part of training before graduation so that these graduates come out ready to be licensed and work whether for the government or private sector. We cannot be training professionals who require internship as a mandatory pre-qualification and yet the universities appear not to care how the people they train are going to end up!

𝗜𝗻𝘁𝗲𝗿𝗻𝘀𝗵𝗶𝗽 𝗨𝗽𝗱𝗮𝘁𝗲!! 𝗜 𝗔𝗺 𝗦𝗶𝘅 𝗠𝗼𝗻𝘁𝗵𝘀 𝗜𝗻 In Nigeria, the internship program for pharmacy graduates is a compulsory, continuous one-year training at PCN-approved centers. It is designed to introduce fresh graduates to professional practices and help them apply their academic knowledge in a practical setting under supervision and guidance. I started my internship in January at Wesley Guild Hospital Unit, Obafemi Awolowo University Teaching Hospital Complex. Now, six months in, it has been an incredible journey of learning and working. With the help of my supervisors, I've learned the different functions of a hospital pharmacist, including: ✨✨ Assessment of prescriptions ✨✨Dispensing medications ✨✨ Patients counselling ✨✨Compounding medications ✨✨ Inventory management ✨✨ And more Monthly rotations through various units, such as geriatrics, antiretroviral, mental health, antenatal, and pediatrics pharmacy, have broadened my knowledge of the specific services and special care pharmacists provide to different patient demographics. I've also participated in several beneficial activities, including outreach to a secondary school, attending and giving presentations. I have presented six times, both individually and in groups, and attended over 14 presentations by registered pharmacists, intern pharmacists, a lecturer, and medical doctors. Overall, my internship experience has been remarkable. While work can be stressful sometimes, these challenges are part of the journey.
