Urgent Call for a More Integrated Approach in NHS Services!! After dedicating a combined 75 years to working in the NHS, my husband and I have experienced firsthand the challenges faced by patients within the system. Recently, my husband was diagnosed with prostate cancer. Shockingly, despite our deep familiarity with the NHS, we had to turn to private healthcare to receive a timely diagnosis. Now, facing another health concern! —we find ourselves navigating a fragmented system. Instead of seeing his consultant, he has been offered an appointment at a different hospital, different consultant scheduled for the beginning of August. This delay and lack of continuity in care highlight a critical gap in our healthcare system. We have spent the last 2 hours working our way around the system! With today’s technological advancements, it is disheartening to see a lack of integration in patient care within the NHS. We are fortunate to understand how to navigate these complexities, but what about those who do not have our background? It is a tough admission to say that the system is broken, but recognising this is the first step toward meaningful reform. We need an NHS that champions not only advanced technologies but also a truly integrated approach to patient care, ensuring that every patient receives timely and consistent treatment. #NHSReform #Healthcare #PatientCare

The Liberty, featured below, is a test-at-home kit that allows cancer patients to take their blood tests and upload the results at home independently. This new device will help reduce the unnecessary time cancer patients spend in hospital for blood work and has since been given regulatory approval. With this intervention, hospital visits are reduced while health professionals suggest it could boost their overall productivity. After being trialed at The Christie, in Manchester, this device will now be deployed at 12 NHS healthcare facilities. 📌 The Christie NHS Foundation Trust, Manchester, manages the Christie Hospital, one of the largest cancer eatment centres of its type in Europe. 📅 On October 3rd, 2024, the PREVENTABLE project held its highly anticipated Midterm Conference and PrevenTalk 2 in Rouen, France. Fineberg chaired the NASEM committee responsible for this pivotal report that aims to bring clarity and consistency to the diagnosis and treatment of the long-term effects of COVID-19, a condition that continues to impact lives globally.   "Despite the fact that there have been many efforts to characterize this condition and many different definitions put forward, there has not yet been a common definition that has been widely agreed upon," said Committee Chair Harvey Fineberg, M.D., Ph.D., president of the Gordon and Betty Moore Foundation. "The lack of a considered uniform definition that's widely accepted hampers individual patients' ability to get the care they need." This was the core basis for Bridge Oncology https://lnkd.in/gB9yVNDF University Hospitals took the right approach to sustainable change. Clinician leadership must drive the initiative. Being cost effective and adding value without compromising quality is achievable and sustainable. Proactive approaches to payer policies and current fee for service models can be reimagined. The reform must be owned and starts with the providers. Too many are focused on the cuts and passing the blame on “control” issues. We have heard this over and over in radiation oncology- prior auth, hypofractionation, MPFS cuts etc. A shift only occurs when you focus on what you can control. Bridge Oncology, like University Hospitals, saw what we could change in the current model. Combine that with a committed team of providers, clinicians, and data and results happen. ✍ VIRTUAL WARDS 💡 A study by researchers at Lancaster University for Wrightington, Wigan and Leigh NHS Foundation Trust and reported in Health Service Journal has found: 💷 £935 - cost of a bed day in a virtual ward compared to a £536 hospital bed 🏥 A clear reduction in length of stay was found - but also higher rates of readmission 🔺 NHS England are continuing with virtual Wards - it's a major policy - and 10,000 already exist with many more to come Caveats: It is a small study and the acuity of the patients needs to be taken into account. The costings may also need to be questioned. Please join us in congratulating attending physician Dr. Ana Sofia Warner on her recent New Faculty Excellence Award from the Boston University Medical Group! In her nomination, her colleagues described her as, "a true bridge-builder and innovator". Dr. Warner created and launched BMC's Hospital Admission Reduction Program (HARP), which provides community-based transitional care to patients 65 and up who have been hospitalized at BMC. 💭 What does “excellence in care” mean to you? For me, it means trying to give the best care to the patients I see in clinic. It also means trying to improve care for patients I may never meet, through programs like HARP and partnerships with different groups and departments. 💭 How is BMC pushing the boundaries of care? I think there are a lot of folks at BMC who are very creative thinkers in terms of how we can engage with patients in our community. I see this every day with the amazing members of the HARP team, they go the extra mile for patients and that makes a big difference. 💭 What does rewriting healthcare mean to you? It means thinking outside the box of how we can engage patients to meet them where they are at. It means not being so tied to traditional ways of treating patients. That’s why our HARP program goes to people's houses. We realized we needed to be proactive after patients were discharged from the hospital, or they would end up coming back into the Emergency Department. 💭 How are you providing equitable and excellent clinical care? I speak Spanish, and I think for some patients it's a big relief to see a physician who speaks their own language. In my work at the Belkin Breast Center, there's a lot of nuance in some of the breast cancer counseling, so I feel like being bilingual is especially important. 💭 What’s special about working at BMC? I get to work with really smart, talented, and dedicated people every day. And we get to try to tackle some really hard problems in health care for patients who are often underserved. Our Annual US Hospital Rankings for Best in Oncology Patient Experience have been released. Link: https://lnkd.in/grbHtpt3 Our rankings are based on an in-depth analysis of over 600 eligible hospitals and clinics nationwide and more than 30 million unique online patient comments. Leading the way is Cincinnati Children's Hospital with a PEP Score of 4.95, followed by runners-up, Piedmont Healthcare and Providence St. Joseph Health. With an average score of 4.91, the key drivers of success among the top 20 healthcare facilities are: • Continuity of Care: Smooth transitions within and across healthcare facilities, and continuity throughout the healthcare journey. • Fast Access: Efficiency and ease of access to healthcare services, including timely appointments and availability of relevant healthcare professionals. • Communication & Involvement: Clear information, communication, and active involvement of patients and loved ones in decisions around their care.

End-of-life discussions are a frequent yet challenging aspect of being a junior doctor. While these discussions are challenging, these conversations provide immense comfort to patients, ensuring their wishes are heard, understood, and shape the care they receive. I recently completed the Cancer Council Australia and Palliative and Supportive Care Education (PaSCE) Goals of Patient Care Conversations course. Reframing a patient's ideas, concerns, and expectations is essential for fostering deeper and more meaningful conversations in the context of end-of-life care.

Becker's Healthcare recently asked C-suite executives from hospitals and health systems across the U.S. who are speaking at its 12th Annual CEO+CFO Roundtable on Nov. 11-14, 2024, to share what their most effective project or initiative in the last year was that yielded great results. Here’s what UH Chief Operating Officer Paul Hinchey MD MBA, MD, MBA, had to say: “We embarked on a multi-year journey to implement our Systems of Care operational model, and we already have some good results to report from the past year. The new approach shifts the view of the delivery model from a site of delivery focus to that of the service line provider to better reflect how the patient experiences the care journey. Changing the operational model allows us to think differently about how we deploy our resources. This includes expanding our virtual footprint, redesigning what care we deliver at home in our Healthy at Home program or making better use of brick-and-mortar assets to better consolidate interdependent services in care and volume hubs. With the focus shifting to the patient journey, we meet their needs first but find efficiency in better use of space, reduced cost of care delivery, higher quality and better outcomes. Two service lines that are well on the way to achieving this are heart and vascular and cancer. For example, UH Lake West Medical Center now has high-level cardiovascular services that had not previously been available there. More cardiovascular subspecialists, a new suite for patients with vascular disease, a new diagnostic testing facility, and more complex interventional, electrophysiology and surgical procedures performed onsite. We've also established three main hubs for cancer care among our 17 sites, in addition to our main campus - UH Minoff, UH Mentor and UH Avon health centers. As a result of this system optimization, the number of participants in clinical trials is on the rise. Participation in smaller-scale trials at community sites has increased by 12% over the same metric in 2022. At the same time, participation in rigorous trials of new drugs or therapies, conducted at the three hubs, is up by 24%.
