The Pterygoid Fixated Arch Stabilization Technique (PFAST) technique provides a means to attain high insertion torque values for dental implants and serves as an excellent full arch technique during certain compromised situations. It is especially useful when treating patients that have been edentulous for quite some time, and could potentially be a candidate for more invasive techniques such as zygomatic implants.  Previously, iliac grafts and bone grafting were used to create suitable bone tissue for endosteal implants, extending treatment time and incurring significant costs for the patient. With subperiosteal implants, the implant base is designed according to the patient’s bone density and the geometry of the alveolar ridge. The dental abutments are positioned based on input from the specialist surgeon and prosthodontist. A crucial aspect of designing the dental abutments is that their position and orientation are tailored to fit the dental prostheses that will be placed on them.   Since these implants are placed beneath the gums and sometimes within the oral cavity, their surface quality is of utmost importance. I’m having dental surgery next week. If you have ever had a tough recovery from an injury or surgery, you know physical stress can be at another level, and healing comes with a lot of uncertainty. Our brains don’t like uncertainty one bit. Yet, we tend to grow most in uncertain times. If you find yourself procrastinating instead of studying for your next exam, it’s time to acknowledge that you might be having difficulty dealing with uncertainty and start studying anyway. No, you can’t 100% know what will come up in the exam. No, you can’t say for sure if you’ll pass at the first attempt. And my personal favorite: No, you don’t need to know whether the BPP or Kaplan books are better. What you already know but need to be reminded of is that the earlier you start studying, the more likely you’ll be to finish the syllabus on time, have some time for rest, and be able to do mock exams. 📣 Start studying for your September 2024 exams now. 60 year old physician who underwent enucleation and curettage of a Glandular odontogenic cysts GOC of his Maxilla, then had a first recurrence and a larger second enucleation and curettage. Later had a second recurrence which resulted in a Subtotal Maxillectomy with a fibula free flap and 7 dental implants (Jaw in a Day). Just two months later, the “JAW In a Day” failed, which resulted in a larger third surgery (second reconstruction), with a large ALT anterolateral thigh flap to obliterate the oro-nasal fistula. He was told he’d never be a candidate for dental implants or any further reconstruction and was forced to use wear a removable appliance until he was referred to me, I debulked the flap and placed 4 zygomatic implants to give him teeth again.
