What do Connected Volunteer Perinatal Peer Supporters do? Well, the list is so varied and very much tailored to the individual receiving support but here are some examples... Visit a pregnant parent at home Facilitate community groups and sessions such as Bumps to Babies and Dads' Space Support a birthing person through labour and birth Visit the neonatal unit to chat to parents Support a parent to attend the hospital for a scan Attend events & champion all things Connected! Support a family with a social care meeting Go for a walk with a new parent Have a phone call with an expectant parent After training, we ask volunteers to commit a minimum of two hours per month to their volunteer role. We support each volunteer to find something they enjoy and that feels right for them. Our volunteers are the heart of Connected Applications for this years' cohort close on Sunday 15th September 2024. Get in touch to find out more...

Under CalAIM, managed care plans may offer 14 Community Supports — services not traditionally covered by Medi-Cal that address social needs. Some of these services, like housing navigation, have been readily adopted, while others have gotten off to a slower start. We recently published a collection of fact sheets that outline implementation tips from organizations offering these less commonly adopted CalAIM Community Supports: 🔹 Asthma remediation 🔹 Day habilitation 🔹 Nursing home transitions/diversion 🔹 Personal care services and respite services 🔹 Sobering centers Check out the collection: https://lnkd.in/g9xkTp_7

As a family caregiver of adult parents myself, I know the role of unpaid caregivers is both vital and underrecognized. The Direct Care Worker and Family Caregiver Initiative from the National Alliance for Caregiving and the PHI group seeks to change that. This Initiative seeks to integrate family caregivers into care teams, develop on-line tools to connect families with paid caregivers, expand access to consumer directed programs, and invest in research into this dynamic. Tapping the true potential of stronger collaboration between paid and unpaid caregivers could be a game changer.
