" I failed my exam by 1.5 marks"- GPST3 This week, an ST3 confessed her disappointment after failing SCA by 1.5 marks. She was let down due to poor time management. In SCA, trainees may take longer due to medical complexity and uncertainty. It's vital to identify the reasons for poor use of time to consult within 12 minutes. My tip is to record your clinic and find out why you are taking longer. The usual suspects are lack of structure, not being willing to commit to a diagnosis or excessive use of summary. Many GP trainees dwell in data gathering beyond 7-8 minutes due to a fear of missing important information. As a result, they don’t leave sufficient time to tackle clinical management. Trainees may have to rush and don’t discuss treatment plans, arrange follow-up or provide safety netting advice. Priming a case before the start of the consultation is critical. Use the three-minute reading time to prepare how you are going to approach the station. Write down key questions you shouldn’t miss to make a safe assessment. Keep track of time during your consultation. Get yourself a timer and keep track of the length of time you spend in data gathering. Aim to progress the consultation and transition to clinical management at minute 6. If the problem is due to a lack of structure. Consider revisiting consultation models and use a crib sheet to organise your thoughts and plan before initiating the consultation. #SCApreparation #MRCGPSCA #SCAexam

Specialist pathway registration - current processing times. Here's our latest update when it comes to important processing times. A number of things to keep in mind: RACGP Part C in-portal submission - at this point you will receive your agreement and invoice for the training program. When you have paid the invoice, and signed the contract, then you don’t need to worry further about the 12 months validity of your Part The Medicare Provider Number - it’s common for doctors to plan to arrive around 1 week before the expected end of the Medicare Provider Number Application Process because they can spend 1 – 2 weeks settling in with orientation, connecting with RACGP and your supervisor, meeting colleagues, learning the IT system, shadowing and getting ready. Please note: the only way that we can make your process any faster is by having the documents ready for the next stage, when you receive certification back from the current stage. Our friendly Doctor Connect team are ready to work with you to help confirm everything that we need is ready for the submittal of the next stage. Doug and I leading an Approved Clinician refresher day today. All medical AC's today, we don't get many multi professional AC's on refresher courses at the moment - but there are many more people training so we're seeing more coming on the Induction courses, always bringing their experience, knowledge and alternative perspectives to the discussions. Today's main topics - Who's responsible when there's no bed? How do we choose between MHA and MCA? Can we treat physical health under the MHA? How long should you keep a CTO in place when someone is stable? Always a lot of practice discussion on these sessions - and between an AMHP, 37 Doctors and a Barrister...there's often some healthy debate! We were lucky enough to be joined for some of the day by an expert by experience too, who's reflections, insights and challenges to us added a depth of perspective we all need. Reflections I asked of the group included: ⚖️ An invitation to consider holding discussions about MHA interview outcomes together with the person and assessing team, where practical - empowerment and involvement in action ⚖️ Recognising that the law often doesn't hold the answers, it's a framework for human compassion, consideration and professional judgement to operate within - use those too! ⚖️ Because of that often it's not just legal advice we need in complex situations...it's practice advice and collaboration ⚖️ Remember....just because the law says we can - doesn't mean we should #MHA #MCA #MPAC #AC #legalupdate #CPD #reflection #AMHP
