More and more studies of questionable quality on the subject of scoliosis and other spinal deformities are being submitted for publication. As a reviewer, I have recently had to reject some of them. Unfortunately, some even make it to publication. This particularly applies to studies on braces. In almost all recent studies on this topic, the brace used is not even illustrated. Since the brace standard is very inconsistent internationally, even within some brace families (e.g. Chêneau brace), it is necessary to present the brace used in the study in detail in words and pictures. It should be standard practice to document an anterior view and a posterior view of each of the braces used for a patient with a thoracic, lumbar and double major curvature pattern. In contrast to Orthobullets last case, I like fixing ligamentous injuries with my plateaus. In fact, i think you can legitamately argue some plateaus are primary ligamental injuries with a concomitant fracture. Generally i find the dominance of US literature on these frustrating. its really hard to filter out the white noise of ACL recons (which seems hardly applicable outside the US as it is) from the strange duality of trauma/sports care. My philosophy with plateau management (even in more unclear cases like this apple-bite with ACL) is restore varus/valgus alignment contain the rim / restore condylar width achieve meniscoligamental stability restore articular surface in that order :) https://lnkd.in/eBEAMevN

https://lnkd.in/gZAu6pxt The classic Gonstead approach to upper thoracic treatment, in my humble opinion, is flawed. While it may work when performed with sufficient thrust force on most patients, it can be harsh on the patient. The main issue lies in the premise that the thrust should be performed posterior to anterior. This is evident in the design of the classic Gonstead sitting chair, which includes a belt to hold the patient’s chest against the back of the chair. A more elaborate approach is described in this video. In this approach, the thrust remains anterior but mostly medial to lateral, leveraging the long spinous process and following the facet plane at this level. This technique offers several advantages: it is much gentler, eliminating the need for a belt to restrain the patient, has a greater predictability for audible release, does not require any special seating, and considers a much more precise tensioning. I will conclude by stating that I attribute to Gonstead my interest in seated technique and my passion for adjusting.
