At HealthSnap, our team is committed to leading with science and using a data-driven approach with our partners, ensuring effective RPM and CCM programs to improve patient care and access. Our latest blog post highlights the effectiveness of the CMS RPM codes, emphasizing their success with engaged patients who regularly transmit data. The heat map featured in the article underscores this. The Y-axis shows baseline systolic pressure from initial measurements, while the X-axis tracks engagement levels, from less than 3 times a week to 6-7 times weekly. Results are compelling across the board, regardless of the starting blood pressure. For example, patients who were most hypertensive with a starting SBP of ≥160 see an 18-point drop with data transmission 3 times a week, and it improves across every level of engagement until a nearly 34-point improvement with those transmitting data 6-7 times weekly, with a significant portion of these patients reaching control levels. These findings demonstrate the transformative impact of RPM programs in managing health conditions when used appropriately and as CMS has intended.
