The Florida Board of Pharmacy is set to adopt amendments to its rule on registered pharmacy technician-to-pharmacist ratios, allowing a supervising pharmacist to determine the appropriate ratio. The current rule requires a 1:1 ratio unless otherwise authorized. The proposed rule removes activity-specific ratios and allows pharmacies to use a ratio greater than 1:1 if they comply with guidelines and the ratio is appropriate. Pharmacists IF the position of a community or retail pharmacist as we know it was eliminated, what would you do? I've been hearing from people who do telehealth and get their prescriptions from a mail order pharmacy. They don't even set foot in a pharmacy anymore. From the consultation to the prescription is all baked into the service. More and more IV wellness places are showing up. There are a few in my area. These places have Nurse Practitioners administering medications (and vitamins) directly to patients. For now its limited to certain therapies and what was once not done at all is now possible. What's to stop the expansion of this scope? More locations, each with their specializations. Some services currently offered include the following medications: * Testosterone Replacement Therapy, hCG, Clomiphene * Semaglutide, Tirzepatide * Ketamine Therapy * Dexamethasone * Ketorolac There are medications that used to be prescription only that are now available Over the Counter. Technicians can now vaccinate I have heard first hand and seen a handful of pharmacists posting about retail models being piloted, eliminating the pharmacist position as we know it. One model with the pharmacist verifying images of the entire process from drop though final verification to sale, with technicians running the pharmacy. Figuring out what I wanted to do instead of pharmacy was my rate limiting step. It took me a long time, a lot of trial and error. Given the climate of today's market and working conditions I feel lucky that I started when I did. Curious to know if any of you spend time thinking about what you would do instead? I think this is the future of retail pharmacy and perhaps many verification positions in long term care and hospital pharmacy. **Remote site verification and counseling.* From a strictly business perspective, pharmacy chains have been dealing with a pharmacist staffing "shortage". In my opinion, there isn't a shortage of pharmacist but a shortage of pharmacist willing to work in abusive and stressful retail environments. Major retailers can now argue, persuasively, that the lack of pharmacist (whether from an actual or perceived shortage) can be solved with technology. Have one pharmacist remotely manage multiple sites with pharmacy technicians on the front lines. From a financial and shareholder perspective, it makes a ton of sense. The major employer for most pharmacist are retail chains. This shift will leave many pharmacist unemployed or underemployed. "If you don't design your own life plan, chances are you'll fall into someone else's plan. And guess what they have planned for you? Not much." Jim Rohn #pharmacy
