The solution for suboptimal statin prescribing: Leverage pharmacists for a 6x improvement! --- A recent study (only an abstract right now--description in the comments) looked to address the chronic suboptimal prescribing of statins for eligible patients by sending them to pharmacists. Eligible patients were automatically identified and referred to a pharmacist for initiation of appropriate statin therapy. A #pharmacist contacted the eligible patients, discussed their personalized risks and the benefits of statin initiation, and initiated therapy when appropriate. That's tough to do in the few minutes that #PrimaryCare physicians have at the doctor's office between all the other (more urgent) issues. --- Patients referred to the #pharmacy were twice as likely to receive a statin (31.6% of eligible patients vs 15.2%) And three times as likely for the statin to be at an appropriate guideline-recommended dose (24.8% vs 7.7%) =SIX times the number of patients receiving an appropriate statin dose compared to the usual care from PCPs. This is not only huge for patient cholesterol management, but also for all the #PopulationHealth quality measures that look at cholesterol/statins. “The findings suggest that automated referrals to centralized pharmacy services for lipid management are a game-changing strategy, drastically improving the odds of prescribing appropriate dose statins.” - Alexander Fanaroff

💊 Patient Safety Above All: Managing Drug Interactions in the Pharmacy 💊 Recently, I encountered a case that reminded me why being vigilant as a healthcare professional is so crucial, especially when it comes to drug interactions. A 75-year-old patient came in with a prescription for clarithromycin, clavulanic acid, and a PPI, having been diagnosed with H. pylori. However, upon reviewing the patient’s medication history, I noticed that he was already taking indapamide for hypertension. 📉 The Risk: Indapamide, a thiazide-like diuretic, when combined with clarithromycin, a macrolide antibiotic, increases the risk of serious heart issues like tachycardia and even cardiac arrest due to QT prolongation. Given the patient’s age and existing hypertension, this interaction posed a significant threat to his safety. 🚫 My Decision: I immediately contacted the prescribing doctor and recommended stopping clarithromycin. Instead, I suggested using Pylera, which combines bismuth, metronidazole, and tetracycline, as an alternative for treating H. pylori without the same risk of cardiac complications. Pylera offers an effective eradication regimen without exacerbating the risk of QT prolongation or interacting with his current blood pressure medication. ✅ The Outcome: The physician appreciated the input, and the patient was switched to Pylera. I felt confident knowing that the patient would receive safe, effective treatment for his condition while minimizing his risk of adverse effects. 👉 Takeaway: Always review patient medications carefully, especially in elderly patients with multiple conditions. Understanding drug interactions can be the difference between safety and severe adverse events.
