Having just read IPAFS Safety report for 2024 and many different IOSH reports for serious and fatal incidents, can you really hand on heart say you are doing everything reasonably foreseeable on your worksite? Now I'm not just talking work at height here however one of the latest reports, the young lad was at height. His hoodie, yes a bloody hoodie snagged in a door mechanism which strangled him! The access ladders used were of the wheeled, handrails and small working platfom on the top, so some good equipment available but a young man lost his life due to a hoodie! Do we plan our job start to finish, terra firma to terra firma? A couple of lessons I have learned from onsite incidents. The remote site ambulance driver only had a pass to get through the 1st gate which allows access to the offices, they couldn't access through gate 2 onto the main site. Luckily the CCR were on the ball and allowed the ambulance access. Another one that involves stretchers, the big orange stretchers don't normally come with lifting bridles and don't go up sea faring vessel steps very easily due to width and how steep stairways are. Look at your tasks start to end folks and make sure we consider all hazards. Involve all your team and clients wherever possible. Stay safe.
