I am honored to have played a small part in a study that gives new insight into the high burden of kidney disease borne by many people of African descent, including African Americans. Briefly, two variants of apolipoprotein L1 or APOL1, a component of HDL (the so-called ‘good cholesterol’), “appear to have evolved to confer protection against the African sleeping sickness parasite, Trypanosoma brucei rhodesiense” (quote from our report), some 4 to 6 thousand years ago. Those same variants, G1 and G2, put people at added risk of kidney disease, likely through damage to renal cells called podocytes. This association was already known, but the mechanism has been elusive. Here, we “establish APOL1-mediated Na+/K+ transport as the proximal driver of APOL1-mediated kidney disease.” Small-molecule drugs that modulate these transporters are under development, fostering hope for new treatments.
