78% of people experiencing homelessness report having a physical health condition. So we were extremely grateful to the Homeless Health team from Central London Community Healthcare NHS Trust who provided drop-in health clinics for day centre and hotel guests over Christmas. They dealt with everything from health concerns and prescription questions to wound care and long-term condition management. Thanks to their incredible care, over 100 of our guests had a happy AND healthier Christmas!

Advocates for Community Health led over 550 health care organizations in urging Congress to reauthorize the Community Health Center Fund (CHCF) in an end-of-year funding package, at a minimum of $5.8 billion per year for at least three years. In a letter delivered to Congressional leaders today, national, state, and local organizations representing all 50 U.S. states and territories outlined the critical need for a robust funding increase for the CHCF, which is set to expire on December 31, 2024, to meet the record-breaking patient volumes and unprecedented financial challenges that health centers are facing. The signers of the letter expressed concerns about the insufficient federal funding to address the increasing needs of communities amidst mental health, substance abuse, and maternal health crises, as well as local emergencies like hurricanes, wildfires, and tornadoes. In the middle of a very busy few days I didn't get a chance to share this Channel 4 News piece about us last week (thanks Matt Downie MBE). Victoria Macdonald 's excellent report shows how a Pathway team with access to specially commissioned step down beds can break cycles of homelessness, help the NHS make best use of that rare resource - hospital beds - and according to new analysis by Alma Economics, if adopted nation-wide would save the NHS and wider public services money. (And of course discharging anyone from hospital back onto the street is surely just plain wrong?) We'll be pushing this as an obvious priority for the new NHS 10 Year Plan.

Discussing the crossroads of hospital care and homelessness. In Victoria Macdonald’s report on Channel 4, Alex Bax voices a sober assessment of the current situation: “in a developed society like the United Kingdom…..we’re seeing large numbers of people discharged to the street; large numbers of people dying on the street…..and we should be better than that” Well said, Alex. It matters to me as this is where two of my worlds collide through the work of NHS England and Crisis. Our hospital wards are full, yet complex discharge needs result in a vicious cycle of care for the homeless. According to the report, if you have no fixed abode you are - 6x more likely to use NHS services - 4x more likely to be admitted - and likely to stay 2x longer in hospital Never has there been a stronger case for step-down care, needing national coordination between key stakeholders and agencies. My hope is that the current work on the NHS 10 Year Plan will find a way to support this most vulnerable patient group.
