Here is a case of previous LSCS, placentation total previa with accreta / increta underlying the scar, at 20 wks GA . USG indicators include loss of retroplacental space, myometrial thinning, increased placental thickness and increased retroplacental myometrial vascularity. Going fwd in pregnancy , bladder wall penetration to be watched for . High risk for APH, PROM and intra op blood loss. Management : planned CS with balloon occlusion and uterine artery embolization, may require hysterectomy.
