In just four years, Atrium Health's care-at-home program has served over 9,400 patients, saving more than 33,000 brick-and-mortar bed days. The care program boasts a readmission rate of only 0.8%, much lower than that of their brick-and-mortar facilities, with a mortality rate below 1%. Their integrated care-at-home approach has not only exceeded expectations but also provided exceptional patient experiences.
