Without Toolbox talks and constant reminders, this will be a daily aspect in our workplaces. The consequence is revenue leakages through expensive aircraft repairs, flight delays, lack of investor confidence, lack of customer trust, sometimes this can be fatal and a worker losing his job. Situation awareness and observing a safety culture in place will help prevent this from happening.

Every incident or accident has multiple contributing factors that we need to consider methodically. No blame culture or positive safety culture will help identify where the system failed and how it can be improved. ICAO annex 13 provides provisions for a methodological investigation process.
