This includes physicians, nurses, and administrative staff, all of whom play vital roles in capturing the nuances of each patient interaction. By implementing standardized documentation protocols, we promote consistency and clarity, reducing ambiguity that could lead to coding errors or misinterpretations of care provided. Dear Caregivers, Families, and Colleagues, As a longstanding mobile medical service on The Treasure Coast, Quality Care Medicine is committed to "Keeping Patients Healthy." Our primary aim is to prevent unnecessary hospitalizations and emergency room visits. One common issue we encounter is patients arriving at the ER without proper paperwork, leaving ER physicians with little choice but to admit them. Often, the lack of family contacts or past medical history exacerbates this problem, leading to avoidable admissions. Having witnessed this "revolving door effect" firsthand across various Assisted Living Facilities (ALFs), it's clear that patients benefit greatly from regular medical care within these settings. Unlike nursing homes, ALFs have limited access to medications and services, making timely interventions challenging when visiting providers are absent. While medical assistants and med techs play vital roles, they lack the expertise to evaluate patients' medical needs, often resulting in unnecessary ER transfers. Providers who actively manage patients within ALFs have a distinct advantage in keeping their patients healthier and avoiding hospital admissions. Let's work together to ensure patients lead healthier, more productive lives by minimizing unnecessary ER visits and hospitalizations.

An automatic nursing assessment system is developed based on a voting workflow to create electronic assessment sheets, a nursing process model (ESR-PGIO), and a multilayer architecture of independent modules.
