Bendigo Community Health Services (BCHS) engaged DJANDAK, a Dja Dja Wurrung enterprise, to design, manufacture and install a welcome sign at their new wellbeing centre for people seeking support with dependency issues in North Bendigo. Aunty Marilyne Nicholls and Jason Kerr were there for the unveiling. The new facility is named Wanyanimbik Wayawan, meaning ‘finding myself’ in Dja Dja Wurrung language, based on the BCHS philosophy of discovery, not just recovery. The DJANDAK team is equipped to help you with your next project from concept through to delivery. www.djandak.com.au 03 5444 2888 djandak@djadjawurrung.com.au Dja Dja Wurrung Enterprises Pty Ltd

Turning Challenges into Opportunities: Optimising the LIFT Estate for Community Health The potential of the LIFT estate is significant yet underutilised. With over 200 modern facilities across England ready to support integrated care, why aren’t we making the most of them? Challenges around costs and occupancy are holding us back, but there are also avenues for change. It’s time to examine these issues critically and consider how we can unlock the full value of these key resources for community health. #LIFT #IntegratedHealthcare #CommunityCare #NHS #HealthInfrastructure #PrimaryCare #HealthcareInnovation #HealthAndWellbeing #FacilityManagement #HealthcareCollaboration #PublicHealth #HealthcareTransformation #DataDrivenHealthcare #PlaceBasedCare #CommunityHealth

Sarah, will you address why Wellcare starting Jan 1 2025 is abandoning thousands of agents and brokers that have serviced and assisted Wellcare members all these years? Cutting off independent agents and agencies nationwide in 2025 will result in massive disruption to those Wellcare members currently enrolled in Part D plans. It is not fair to Seniors who rely on their local broker to guide them and assist them. It is not fair to the brokers who work hard to help them and will now be forced to turn away those same Seniors and inform them they are "on their own" for service and support. Pulling the rug out from your agent and broker community has sent shockwaves through the industry. You really should talk with Wellcare's CEO Michael Carson and direct him to retract the recent decision to eliminate all compensation for agents and brokers on part D plans, including new AND renewal policies. Is it worth alienating the massive broker community to save the company a few bucks (less than $5 per member per month)?
Community health centers (CHC) are the heart of community-based care, offering personal, high-quality care deeply rooted in the neighborhoods they serve. I deeply appreciate the work that the National Association of Community Health Centers (NACHC) and its members do every day to help the communities we both serve. Last week, I had the opportunity to attend NACHC’s 2024 Community Health Institute & EXPO and discuss the work that its members do every day and our shared goal of improving community health. Thank you Kyu Rhee, MD, MPP for the great conversation! #CHIexpo24 #ValueCHCs #Healthcare #CommunityHealth

🫂 Te Puna Oranga o Ōtaki delivers our youth development in Otaki - though collaborations and social investment, including from Council. 🫂 This is a way our council supports our community - not by direct delivery, but by partnering with community to deliver the impact where it's needed. Community development, right? 🫂 It's inspiring to see the ALL the services provided by Te Puna Oranga o Ōtaki in the video clip, including the newest arm of a new GP service - for the people of Otaki, by the people of Otaki. 🫂 Nā tō rourou, nā taku rourou, ka ora ai te iwi (With your food basket, and my food basket, the people will thrive)

Robust international evidence demonstrates that #CHWs are vital to achieving #healthforall. But despite their overall success, not all CHW programs are equal. CHW AIM is a multi-purpose tool for community health program design, assessment, and optimization. It can be used for the implementation of new programs, to evaluate and improve existing services, and to promote professional CHW (#proCHW) policies and practices. In North Lanarkshire, we understand the power of collaboration and multi-agency working in delivering the highest quality of care and support to those who need it most. Great energy in the room today for the Locality Planning Group development day for Motherwell Locality. Health and social care services are complex and multifaceted, and no single organisation or service can address all the needs of all individuals and communities alone. That's why we should remain committed to working together across agencies, disciplines, and sectors to provide seamless and integrated care that truly meets the diverse needs of our residents. By collaborating with partners in health, council, social care, 3rd sector, housing, and beyond, we can: ✅ Pool resources and expertise to provide holistic support to individuals and families. ✅ Identify and address gaps in services, ensuring that no one falls through the cracks. ✅ Foster innovation and best practice through shared learning and knowledge exchange, especially by listening to those who use services and their carers. ✅ Empower individuals to access the right care, at the right time, in the right place. At the heart of our collaborative efforts is a shared commitment to putting the needs of people who use services first. Together, we can break down silos, bridge gaps, and build a stronger, more resilient community where everyone has the opportunity to thrive. #Collaboration #MultiAgencyWorking #HealthAndSocialCare #NorthLanarkshire #CommunitySupport
