It’s a privilege to be part of a team that shares my commitment to providing the care these young patients truly need. Over the past year or two, I’ve seen an increase in the number of children on my schedule who have been previously papoosed for dental work, often carrying significant trauma as a result. In every pre-op evaluation, I make it a point to discuss the child’s dental history with their parents, particularly the decision for general anesthesia. These conversations open doors that might otherwise stay closed and create an opportunity for me to connect with the child on a deeper level. I also ensure my team is aware, and together we adjust our approach, slowing down to make the child feel more at ease before induction. 🔹This is Adam too. Severe cerebral palsy, result of premature birth. Referred at age 16. Cognitively disabled with little function. Increasing nursing demands, increasing limb deformities. The goal: make his day-to-day care easier & improve his quality of life. 🔹18 months after the #TDD procedure: 1️⃣ General improvement but baclofen has only been a part of it. A tracheostomy was necessary & he had a PEG inserted. Gained weight, has a more relaxed expression. 2️⃣ Has more muscle bulk & fat cover over the entire body. Limbs move with greater ease. The contractures remain. Abduction of the legs is possible, a function essential to proper care. 3️⃣ The ankle can be moved within a restricted range where before the implant the ankle was entirely fixed. Displacement of the ankle bones. 4️⃣ Legs easily parted with a major impact on nursing care & quality of life. 5️⃣ Passive movements are done with ease. Contractures remain. 6️⃣ Hardly any gain in range of motion in the upper limb. Movement is easier. Once delicate & complex joints (wrist, ankle) are gripped, progress is slow or non-existent. 7️⃣ The contractures of the wrist have stopped the fingers from being functional. Displacement of wrist bones (carpal laxation). Orthopedic surgery can correct this deformity but may not improve the function. 8️⃣ Sitting balance, which was adequate before the implant, did not improve. His head droops more. 🔹 Spasticity shows itself differently in every child. Some children use the spasticity to stand, sit up straight or control their head & neck (#functionalspasticity). Baclofen has an effect on the entire nervous system & although the tightness improves, the functional spasticity may also decrease leading to unwanted changes.
