Get it RIGHT, make it SAFE The slogan for this year's Patient Safety Week at FV Hospital takes me back to the early days of my medical studies, with the reminder: Primum non nocere – First, do no harm! This reminder from the father of medicine, Hippocrates, aligns with the fundamental philosophy of Buddhism as well as other religions: "Do not do to others what you do not want done to yourself." At FV Hospital, we strive together to fulfill our commitment not only to diagnose correctly and successfully treat the patient's illness, but also to ensure that this is done safely, so the final outcome is recovery and an improved quality of life for the patient. Recently, I was frustrated to hear a conversation between a seasoned Crit Care Medic and a brand new Crit Care Medic where the seasoned one told the new one something along the lines of Crit Care being super easy and that you can ignore most of what's going on with the patient. 99% of drips you can set and forget, you don't need to know anything about them, what they do, how they operate, etc. This same line of thinking was then applied to pretty much every aspect of Crit Care. I could see the passion the new CCP had leave his eyes as the convo was taking place. Frankly, if you agree with the seasoned Crit Care Medic, it's time for you to leave the Crit Care side of the field, and likely the field altogether. I love Crit Care, delivering high quality Crit Care has become my passion within EMS. It's complex care with many puzzles to solve and solutions to problems waiting to be found. If you care so little about what you do that you seemingly take joy in stamping out the passion others have for the job, you're part of the problem, not the solution. I wish I could say this was an isolated incident, but sadly this attitude is one I encounter far too often on the ground side of Crit Care. I don't have the answer to the why or the definitive method of addressing this issue. But, if we want high quality Crit Care clinicians the approach of the seasoned Crit Care Medic in this story is not the path forward and is something we should actively push back against.

Taking care of patient at home or home care principally is not to move all medical equipment or health workers from the hospital/clinic to the house. It provides a high quality of comprehensive care with holistical approach to the patient in a limitation of medical care. Holistic here means considering much on the basical needs of patient. Not only medicine but also emotional or psychological and the comfortness of being at home with the family. That is why family doctor plays an important role since he knows the characteristic of the patient and his family well as part of his job. Due to the limitation, the multi intervention should perform in an evidence based medicine combine with experience in daily practice way. Considering it may trigger some concequences, even the worst one, it should be communicated and cooperated well by the doctor to the patient and his family. Every few days I get a message announcing the arrival of senior doctors from the public sector joining the private sector. One thing that struck me is the majority are from the ‘regional’ hospitals. We often hear the phenomena listed below from doctors, especially when lecturing to groups of doctors. Particularly those in private practice and "hospitals" worldwide. We hear similar responses from dedicated doctors. Although it is not taught as depicted in the picture below, it reflects the reality. For example, when a doctor (or a group practice) begins advising or prescribing higher doses of vitamin D (usually after our lectures and justifications), the need for patients who are taking higher doses of vitamin D (and magnesium) to revisit the doctor's office gradually decreases. So, fewer patients visit doctors' offices! What are your thoughts?
