Responsible Dialogues: How we speak about a topic, how we portray it in images- frames our thought towards it. IFSO International Federation for The Surgery of Obesity and Metabolic Disorders has always been working towards a passionate advocacy for persons with obesity. The compassion towards the people living with the disease along with the maintenance of a scientific approach to obesity treatment has always been the motto. A new directory of acceptable terms in the obesity literature is a big leap in the direction. Along with the kind of imagery that the journal no longer finds acceptable- (no stereotypical/stigma: fast food, ill-fitting clothes, etc.). Kudos! Biomed News update on mal de débarquement syndrome (MdDS): 1. Nagliya and Daryanani present a case of a patient with presumed diagnosis of MdDS with a rocking sensation. Note that the case seems to deviate from the Barany Society’s diagnostic guideline criteria in that the patient reported experiencing false motion sensations that occurred in episodes of only 30-60 minutes, following which he felt "off" for the rest of the day. The diagnostic guidelines, in contrast, indicate the presence of non-spinning vertigo to be continuous or for most of the day. A temporary reduction in symptoms with re-exposure to passive motion (a distinguishing feature of MdDS) is also not described in this report. 🎉 𝐏𝐮𝐛𝐥𝐢𝐜𝐚𝐭𝐢𝐨𝐧 𝐚𝐥𝐞𝐫𝐭 Our group member Vid P. and colleagues have just published a new article in Bone entitled: 𝑩𝒐𝒏𝒆 𝒉𝒆𝒂𝒍𝒕𝒉 𝒊𝒏𝒅𝒆𝒙 𝒊𝒏 𝒕𝒉𝒆 𝒂𝒔𝒔𝒆𝒔𝒔𝒎𝒆𝒏𝒕 𝒐𝒇 𝒃𝒐𝒏𝒆 𝒉𝒆𝒂𝒍𝒕𝒉: 𝑻𝒉𝒆 𝑮𝒆𝒏𝒆𝒓𝒂𝒕𝒊𝒐𝒏 𝑹 𝑺𝒕𝒖𝒅𝒚 Bone acquisition trajectory in the paediatric population is an important determinant of peak bone mass at young adulthood. Low peak bone mass is a risk factor for osteoporotic fractures later in life. Dual x-ray absorptiometry (DXA) is used to measure overall bone health, yet it is not used often due to certain limitations. That is why Vid P. explored whether bone health index (BHI), a measure acquired from hand radiographs, can be used to asses the risk of paediatric fracture, how well does it relate to overall paediatric bone health, and the genetic background of the measure itself. 🩺Neuroleptic Malignant Syndrome #NMS 🌟The biologic basis of #NMS is not known, but neuroleptic antipsychotic agents block dopamine D2 receptors, a family of G protein–coupled receptors that bind extracellular dopamine, and this blockade is inferentially implicated in the disorder. 🌟The typical presentation of #NMS is: 🔸#Dysautonomia, particularly tachycardia and rapidly fluctuating hypertensive or hypotensive blood pressure. 🔸 #Hyperthermia to 40°C or higher. 🔸 #Delirium that in the severe form is catatonia. 🔸 #Rigidity. 🌟 The medications that are associated with #NMS include high-potency, first-generation (typical) #antipsychotic agents such as haloperidol, fluphenazine, and pimozide. The syndrome is less common and less severe with second-generation agents than with first-generation agents. Other drugs that block dopamine, such as metoclopramide, droperidol, prochlorperazine, and dopamine-depleting agents such as tetrabenazine. 🌟 Differential diagnosis includes other states of rigidity and hyperthermia, most importantly, #Acute_serotonin_syndrome which may cause acute dysautonomia, although hyperreflexia, clonus, myoclonus, and shivering, which characterize serotonin syndrome, are not components of #NMS. The muscular salience in serotonin syndrome is closer to spasticity than to the lead-pipe rigidity of neuroleptic malignant syndrome, and hyperreflexia is characteristic of serotonin syndrome, in contrast to the diminished or normal tendon reflexes in #NMS. 🌟Management of #NMS requires a tiered approach to the main features of the syndrome: blood-pressure instability, hyperthermia and rhabdomyolysis from severe rigidity, and the potential for respiratory compromise. Recovery times vary but generally range from 7 to 11 days and may be roughly predicted from the half-life of the implicated neuroleptic drug. #synapse #NeuroCritical
