After braving the train strikes and defaulting to the coach network I finally made it to chair an important panel discussion on "creating a culture of systems transformation" at the Public Policy Projects ICS Delivery Forum held last week in Birmingham. This was a gathering of stakeholders from across the health and care system to assess progress to date on the newly created 42 Integrated Care Systems (ICSs) recently set up to facilitate the more holistic improvement of population health across the different geographical regions across England. My excellent panel featured a great mix of perspectives and included: Warren Heppolette, Chief Officer for Strategy and Innovation at NHS Greater Manchester Integrated Care, Katrina Percy, Joint CEO of the NAPC and Helen Hughes, Chief Executive of Patient Safety Learning. The creation of ICSs was based on a fundamental recognition that improving the health of a population can't be delivered by health silos working in isolation.... it is going to be the transition to more systems based and more holistic approach to care that will bring about the improvements in population health that is most desperately needed. "#Culture eats strategy for breakfast" as the saying goes so it was great to explore some of the cultural challenges to more systems based working. Warren gave a the perspective from a whole systems level, while Katrina provided the primary care and #neighbourhood working perspective while Helen brought the patient/citizen perspective as the ultimate beneficiaries of ICS and wider Integrated Care Partnership (ICP) working. Some key insights included: - Without a longer term "systems based economics" and funding model we will continue to fund, target and incentivise short term silo-based behaviours. - Without a shared long term vision we will continue to invest in the pieces of the jigsaw without a shared understanding of the collective picture we are trying to build together. - A deeper understanding of individual and community needs is required so we can co-create the integrated services that really matter instead of fitting people into our services. - We need to create the conditions that release the curiosity, care and compassion across frontline health workers and neighbourhood teams such that they feel safe and inspired to contribute to the collective improvement and innovation of care. - We need to create a shared identity and sense of purpose across ICS/ICP teams to inspire a greater sense of belonging and shared commitment to collaborative and partnership-based working. Stafford Beer, the famous systems thinker, noted that "the purpose of any system is what it does"—meaning if the system isn't delivering what’s needed, it’s not the people's fault but a flaw in the system's design. #Health and #care are riddled with silos and fragmentation. Specialists, services, and apps address isolated parts of our bodies, but no one looks after us as whole persons, within the context of our lives. As society ages and grows sicker, we find ourselves bouncing between disconnected specialists, clinics, and "pathways," often unaware of our histories or other care we're receiving. Integrated Care has long been discussed but rarely delivered. The "sausage machine" model of #healthcare inherited from industry keeps optimising parts, not wholes. Demand rises, resources are strained, and waiting lists grow. Integrated Care Systems (ICSs) were conceived as a new model to bring together health and care into a more holistic approach, supporting wellness, early intervention, and person-centred care for disease or infirmity. For an #ICS to be more than a rebranded silo collection, it must serve as a system-level convener of integrated support, fostering collaboration across health, social care, local councils, communities, VCS, and industry partners. How do we shift from siloed services to holistic support that centres on people’s needs and lived experiences? And how can we view this challenge through a life course lens—Start Well, Live Well, Age Well, and Die Well? Transforming silos into systems requires a shared vision for the future and a collective journey to reach it. One that recognises that it is the relationship BETWEEN the parts that creates a strong and effective system. I’ve been honoured to work with Suffolk and NE Essex (#SNEE) ICS on their breakthrough “Future Shift” programme https://shorturl.at/RCKBM. This unique 3 Horizons programme has united leaders across the ICS to reimagine a future holistic SNEE system and the journey to get there, built on a shared identity and a "Can Do Health & Care" ethos. Stepping back to view the national landscape, we need more of this visionary, systems-based approach—not just additional funding to sustain outdated methods—if we are to build the future health system that we all need and deserve. The NHS needs modern, safe places where staff can give patients first-class care. The LIFT portfolio is modern and delivering for the NHS already. Over 300 health centres were built in local communities, largely in areas of high health need to support more integrated care in neighbourhoods. More diagnostics being delivered locally is a key way in which CHP can support the shift from hospital to community. To support the drive for productivity and value across the LIFT portfolio we look at buildings holistically, considering optimal use and driving up overall space utilisation, this is where greater value and impact can be achieved. Lord Darzi’s independent investigation into the NHS in England diagnosed a health service that was in critical condition but that its vital signs were strong. The province has recently invested funds and pledged that every Ontarian will have a primary care provider, leveraging team-based care, within five years through the leadership of Jane Philpott as Chair of the New Primary Care Action Team. To realize the full potential of primary care teams, the system must prioritize education, coaching, and continuous support for both new and evolving teams. Working collaboratively--whether as a co-located team or distributed network--requires thoughtful and deliberate effort. CACHE, with over 15 years of expertise in interprofessional collaboration, offers a flexible suite of professional development programs, coaching, and customized consultation services. These national and international offerings derive from the scientific evidence base for team-based and collaborative care and have successfully supported over 4 000 health care providers and teams.
