AJOG Expert Review in Labor: Pharmacologic and nonpharmacologic options for pain relief during labor: an expert review - Transmission of labor pain. Labor pain has a visceral component and a somatic component Noxious impulses from the uterus and cervix follow afferent sensory-nerve fibers that accompany sympathetic nerves, traveling through the paracervical region and the pelvic and hypogastric plexus to enter the lumbar sympathetic chain and the dorsal horn of the spinal cord through the white rami communicantes of the T10, T11, T12, and L1 spinal nerves. Noxious impulses from the vagina and perineum travel via the pudendal nerve to enter the spinal cord at S2 to S4. Reprinted with permission from Eltzschig HK, Lieberman ES, Camann WR. Understanding Rhinitis Medicamentosa: The Rebound Effect of Nasal Decongestants As an otolaryngologist, one of the common yet often overlooked conditions I encounter is Rhinitis Medicamentosa—a condition caused by the overuse of nasal decongestant sprays. Often referred to as the "rebound congestion" phenomenon, Rhinitis Medicamentosa occurs when patients use topical nasal decongestants (like oxymetazoline or phenylephrine) for an extended period, typically beyond 3-5 days. Instead of improving symptoms, the nasal lining becomes dependent on the medication, leading to persistent congestion, inflammation, and even structural damage to the nasal passages. What’s key for both patients and physicians to remember is that short-term relief can lead to long-term harm. Educating patients about proper decongestant use, identifying underlying causes of chronic congestion, and providing alternative therapies like saline irrigations or prescription nasal steroids can make all the difference in managing this preventable condition. Rhinitis Medicamentosa is a clear reminder that more is not always better when it comes to treatment. Let's continue raising awareness on the responsible use of nasal decongestants for improved patient outcomes! #RhinitisMedicamentosa #ENT #Otolaryngology #ReboundCongestion #NasalHealth #PatientEducation #ResponsibleMedicine #ENTAwareness
