Transcatheter aortic valve replacement (TAVR)........ Transcatheter aortic valve replacement (TAVR) is a procedure to replace an aortic valve that is narrowed and doesn't open fully. The aortic valve is between the left lower heart chamber and the body's main artery. Narrowing of the aortic valve is called aortic valve stenosis. The valve problem blocks or slows blood flow from the heart to the body. TAVR is minimally invasive, which means it uses smaller incisions than open-heart valve surgery. It may be an option for people who can't have heart surgery to replace the aortic valve. TAVR can help reduce chest pain, shortness of breath and other symptoms of aortic valve stenosis. The decision to have TAVR is made after talking with a team of heart doctors and heart surgeons. The team works together to determine the best treatment option for you. A favourite #cardiology tool on MDApp is the Detsky Cardiac Risk Index calculator that assesses #perioperative risk of cardiovascular events in in patients scheduled for major or minor noncardiac surgery and provides an automatic result interpretation. Preoperative risk assessments are currently compulsory, as a mean of avoiding any undiagnosed conditions, especially of cardiac nature. The earlier the identification of patients who are stratified in high risk groups, the better patient management can be put in place, along with analysis of the benefits and drawbacks of surgery. Each of the 8 items in the Detsky #cardiacrisk index is weighted according to its individual predictive value. The sum of points awarded to each item, as described in the previous section, makes the total score, which ranges between 0 and 85. The higher the score, the greater the #cardiovascularevents risk. Intravascular Ultrasound (IVUS) IVUS is a diagnostic procedure using sound waves to visualize the inside of blood vessels, particularly coronary arteries. A catheter with a tiny ultrasound wand is inserted through the groin, providing an inside-out view of artery walls. This helps evaluate the need for heart surgery, treat certain heart conditions, and assess stent placement during angioplasty. IVUS reveals cholesterol deposits and helps understand stent restenosis. Risks include general anesthesia complications and potential damage to heart structures. After the procedure, a brief hospital stay may be required, depending on whether it was done during cardiac catheterization or angioplasty. How nice when you have the chance to use thromboatherectomy and IVL devices for the same patient. This patient is elderly obese, diabetic, heart failure, CAD candidate for CABG, complaining of severe rest pain. CFA Recanalisation by #Rotarex then IVL by #Shockwave balloon followed by DCB. Distal SFA lesion angioplasty was performed on the way. The CFA suboptimal result was accepted without leaving material behind us, it was enough to subside dramatically his pain. Usually, our routine in such cases is hybrid procedure by femoral endarterectomy, patching and SFA angioplasty. The percutaneous procedure was performed in ambulatory to avoid any SSI or delay of his cardiac surgery. #RotarexS #IVL #Shockwave balloon #KUWAIT
