“Ordering a coronary computed tomography angiography (CCTA) exam is the most effective first step when evaluating patients with stable chest pain, according to new research being presented at the American College of Cardiology (ACC) Cardiovascular Summit 2024 meeting in Washington, D.C. CCTA has gained significant momentum in recent years, especially after updated chest pain guidelines released in 2021 by the ACC and American Heart Association gave the modality a Class 1A recommendation. CCTA is also noninvasive, widely available and relatively cheap compared to some other imaging modalities, making it even more appealing to heart teams throughout the United States. Another factor that appears to be in CT’s favor is the fact that artificial intelligence models are already available that can help evaluate patients with suspected coronary artery disease (CAD)”

🚨📰: 𝐂𝐓 𝐅𝐢𝐫𝐬𝐭 𝐒𝐭𝐫𝐚𝐭𝐞𝐠𝐲 𝐟𝐨𝐫 𝐂𝐡𝐞𝐬𝐭 𝐏𝐚𝐢𝐧 - A study presented last week at the American College of Cardiology Cardiovascular Summit provided reinforced support for Coronary CTA's guideline recommended role as a first line test for Chest Pain. 🔬: 𝐑𝐞𝐬𝐞𝐚𝐫𝐜𝐡𝐞𝐫𝐬 𝐀𝐧𝐚𝐥𝐲𝐳𝐞𝐝 - 786 Atrium Health patients who had no prior diagnose of #coronaryarterydisease and underwent invasive coronary angiography. Patients who’s pre-ICA testing started with CCTA led to far more revascularizations (62%) than patients who received Stress MRI, Stress Echo, No Tests, or SPECT (50%, 40%, 34%, 31% respectively). #cad #cardiology #cardiacimaging #hearthealth #CCTA #cathlab

Our Meta-Analysis titled "Influence of Previous Coronary Artery Bypass Grafting on Clinical Outcomes After Percutaneous Coronary Intervention: A Meta-Analysis of 250,684 Patients" has been published in Clinical Medicine Insights: Cardiology by Sage Key Takeaways: • Worsened Outcomes: Patients with a history of CABG undergoing PCI had significantly higher short-term and long-term mortality rates, increased risk of myocardial infarction, major adverse cardiovascular events, and procedural complications like perforation. • CABG-naive patients were found to have a greater risk of cardiac tamponade and the need for repeat CABG, but no significant differences in stroke, bleeding, revascularization, or repeat PCI were noted between the groups. • These findings underscore the need for refining procedural and surgical techniques and fostering continued research to improve outcomes in patients with prior CABG history. 🚀 Breakthrough in Next-Day Discharge for TAVI Procedures! 🚀 This week, ArchCath wants to spotlight a recent study published in The American Journal of Cardiology 🫀. It has emphasized the feasibility and safety of next-day discharge following transfemoral transcatheter aortic valve implantation (TAVI) using the Acurate neo/neo2 self-expanding aortic bioprosthesis. The study, conducted at the London Health Sciences Centre, Western University, evaluated 368 patients and found that next-day discharge (NDD) did not increase the risk of death or rehospitalization within 30 days post-procedure. 🔍 Key Findings Include: • 📉 Low Readmission Rates: Comparable readmission rates across patients discharged within 24 hours, 24-48 hours, and those discharged later. • 🛡️ Safety: No increase in complications for NDD patients, with a 1.4% stroke rate limited to those discharged after 48 hours. • 📈 Efficiency: NDD patients showed excellent recovery metrics without additional risk. 👆 This research is a significant step forward in optimizing TAVI procedures, improving patient outcomes, and enhancing recovery times. 📃Scientific paper: Is Crash Loading Acceptable in Carotid Artery Stenting? Abstract: Purpose In elective carotid artery stenting (CAS), antiplatelet therapy (APT) is crucial. Several international societies have provided guidelines for loading time and dosage in endovascular treatment; however, no recommendations have been made for urgent, nonthrombectomy-associated CAS without adequate loading time. Here, we investigated the short-term outcomes for APT-naïve patients receiving “crash loading” (CL) on the day of intervention, compared with those for patients wi APT onset 3–5 days (semi-CL) or more than 5 days before CAS (EL). Methods Outcomes of patients 30 days after CAS were evaluated in terms of the rates of in-stent thrombus, re-stenosis, stroke, hemorrhagic and thrombotic events, other periprocedural occurrences, in-hospital death and CAS-associated death. Patients’ biological, pathological and hemostatic factors were recorded and compared. Results A total of 1158 patients who received CAS at the authors’ neuroradiology institution were analyzed: 275 EL, 846 semi-CL, and 37 CL. The patients receiving CL had the lowest rate of stroke, but the highest rates of CAS-associated and in-hospital deaths, although the deaths were not necessarily associated with APT. In-stent thrombosis was the highest in the semi-CL group. The rates and types of periprocedural occurrences favored the CL group.
