The news this week that Fairview Health Services cut 50% of its staff chaplains is disappointing. As leading healthcare providers across the country are finally investing in #socialdeterminants, finally investing in interventions that address #healthinequities, and finally investing in the shift toward #vbc, it’s unfortunate and frankly confusing that one of the Twin Cities’ flagship healthcare institutions is disinvesting in members of the indersciplinary team that research consistently demonstrates: 🟰Advance culturally appropriate care 😁Improve patient experience ❤️Improve health outcomes What happened to 21st century medicine being person-centered and taking a bio-psycho-social approach? What happened to 21st century medicine rooted in evidenced-based medicine? Fairview’s statement in the Star Tribune article, “We will continue partnering with our region's diverse community of faith leaders and cultural healers to support our whole-person approach to health care now and in the future” conflates the role of clinically trained and experienced chaplains with community clergy. While community faith leaders are an important source of support for a person’s wellbeing, they are not a substitute for professional chaplains who are clinical peers to other interdisciplinary team members. They are not a substitute for chaplains who are trained and experienced in providing health care services in healthcare settings and who have deep experience in addressing existential distress within a clinical care plan and clinical context. We are faced with yet another unacceptable example of historical policy decisions driving health inequities that disproportionately impact communities of color. As stated by Filaine Deronnette, Compact leader and Vice President At-Large of 1199SEIU United Healthcare Workers East, “Steward Health Care’s decision to close Carney Hospital and Nashoba Valley Medical Center is devastating, which will create hospital deserts and exacerbate health inequities in the communities they serve. Patients across Massachusetts will now ultimately pay the price of Steward’s mismanagement if care is pushed miles away and ER capacities reach crisis level.” When key providers shut down, those marginalized most lose access first, intensifying the strain on our entire health system and exacerbating health inequities. Yesterday CommonWealth Beacon published my perspective on the evolving Steward Health Care crisis in Massachusetts. As our state leaders contend with the many uncertainties ahead and influence the future of these critical access points, #healthequity and health equity leaders must be at the center of the discussion. Amidst the turmoil, we have the opportunity to impact three important determinants central to health: 1. Creating and maintaining opportunities for health - economic stability 2. When I was enrolled in my BSN program, we learned about the two goals of Healthy People 2010- to increase quality and years of healthy life and to eliminate health disparities. Then came the four goals of Healthy People 2020 - to attain high-quality, longer lives free of preventable disease, disability, injury, and premature death; Achieve health equity, eliminate disparities, and improve the health of all groups; Create social and physical environments that promote good health for all; and Promote quality of life, healthy development, and healthy behaviors across all life stages. (CDC.gov) We’re now up to Healthy People 2030 and there are five overarching goals. In 2020, there were over 124,000 residents of the Rockaway peninsula, and one ER that handled over 37,000 visits (Peninsula General closed in 2012). The nearest trauma center, a good 20+ minutes away, saw almost 85,000 patients. And the nearest pediatric trauma center is even further away. The peninsula’s residents deserve better. Let’s finally put a dent in the healthcare disparities that exist in our own backyard. #SDOH #HealthyPeople2030 #Healthcaredisparities

10 weeks.... 10 weeks today since a GP referral was marked as 'urgent and debilitating' to a private specialist in the Northern Territory, for a patient with a serious health background, and who has been struggling for six months with even usual activities. This person is my family member. 10 weeks today and not even a call, letter, acknowledgement, or waitlist (even when private). I have spent more time that I care to admit talking to providers from around Australia, and whilst they are all under the pump, none are this bad. Where total disregard is the status quo. Two flights and two specialists later, back in Darwin. At our families own cost, having travelled interstate for care. The question must be asked. What do be compromise to live in the Northern Territory, and what are we willing to compromise? And what of those who cannot afford to do so? Or who do not self, or, have family advocates who will push? Is living in the NT only for those who are prepared to forego care, wait until it is acute/urgent, or can get on a plane? I have a first-hand understanding of the challenges that exist working in Health. It is hard. But the gaps that go unchecked are significant. There is space for improvement, but that takes decision making and commitment - to do things differently, to find what works. This can't be the best we can do. We can do better.
