#EHRA2024 is coming to an end. #Newoptions in VT - Cardiac Radioablation is gaining acceptance within the EP community. STereotactic Arrhythmia Radioablation and its place in modern cardiac ElectroPhysiology: Results of an EHRA Survey A total of 129 international participants completed the survey. Based on their knowledge, up to 59% of survey participants responded they would consider performing / referring a patient for STAR. Reported main advantages of STAR were efficacy in the treatment of arrhythmias not amenable to conventional treatment (49%), non-invasive treatment approach with overall low expected acute and short-term procedural risk (23%) and possible higher efficacy than other available treatment options (23%). Most respondents foresee a future clinical role for STAR in the treatment of VT/VF with or without underlying SHD (72% and 75%)!

Abstract In the 2000s, cardiac resynchronization therapy (CRT) became a revolutionary treatment of heart failure with reduced left ventricular ejection fraction (HFrEF) and wide QRS. However, about one-third of CRT recipients do not show a favorable response. This review of the current literature aims to better define the concept of CRT response/nonresponse. The diagnosis of CRT nonresponder should be viewed as a continuum, and it cannot rely solely on a single parameter. Moreover, baseline features of some patients might predict an unfavorable response. A strong collaboration between heart failure specialists and electrophysiologists is key to overcoming this challenge with multiple strategies. In the contemporary era, new pacing modalities, such as His-bundle pacing and left bundle branch area pacing, represent a promising alternative to CRT. Observational studies have demonstrated their potential; however, several limitations should be addressed. Large randomized controlled trials are needed to prove their efficacy in HFrEF with electromechanical dyssynchrony.

Case base learning with Dr Narayanan Thulasidasan at Guy's and St Thomas' NHS Foundation Trust, he discusses measuring the vein accurately, aiming for a clear diameter measurement, with attention to not include any residual thrombus. The measured vein has a minimum diameter of 10.4 mm and a maximum diameter of 14 mm, leading to an average estimate between 12.5 mm and 13 mm. Based on the measurement, a 14 x 150 mm Abre stent is chosen for the procedure. The plan includes post-dilating the distal end of the stent up to 12 mm, depending on how the transition from the stented to unstented section looks via IVUS. Decisions regarding the procedure are made dynamically, based on real-time imaging feedback and clinical judgment during the intervention. 🫀 Patient: 1st time ablation for persistent #afib with Dr. Das at Indiana University Health. Two ECGs were analyzed with vMap, and both showed hot spots on the left atrium anterior septal. 🔎 A #CARTO map revealed heavily fragmented spots that aligned in the same area as vMap. Dr. did PVI followed by anterior line connecting from right pulmonary veins to mitral annulus. 🎯 Pt. cardioverted to sinus rhythm; afib was non inducible afterwards. #clinical #cardiology
