🔥 Ablation of atrial tachyarrhythmias in patients with an extracardiac conduit Fontan 💔 can be challenging due to access issues to the pulmonary venous atrium (PVA). Transcaval cardiac puncture (depending on cavoatrial overlap) or direct conduit puncture can be performed. An alternative is a retrograde approach using remote magnetic navigation (RMN) 🧲 and a flexible ablation catheter (Navistar RMT Thermocool). Using this technique, we recently successfully ablated a focal AT from the coronary sinus in an adult patient with an extracardiac conduit Fontan (Figure, with permission of patient). The catheter goes through aorta, LV, PVA, remnant RA and CS. This case shows the unique capabilities of RMN in treating patients with complex congenital heart disease.
