In this episode, host Dr. Ally Baheti interviews interventional radiologist Dr. Saher Sabri and vascular surgeon Dr. Frank Arko about their perspectives on a July 2023 New York Times article about the ethics of peripheral arterial disease (PAD) treatment in outpatient-based labs (OBLs) and ambulatory surgery centers (ASCs). Dr. Arko emphasizes the importance of approaching articles as an unbiased reader and notes that the article fails to mention that most practitioners follow society guidelines to provide appropriate and effective treatment for their patients rather than prioritizing financial gain. Dr. Sabri believes that PAD and critical limb-threatening ischemia (CLTI) are not very well known by the general public, and this article was a missed opportunity to bring attention to these conditions and disparities in treatments depending on patients’ geographic locations and ethnicities. The article may have the effect of scaring patients away from seeking treatment for PAD and CLTI, as well as receiving care at OBLs, which were originally founded to make interventions more efficient and patient-friendly. We recently had the privilege of experiencing an academic-style lecture and demonstration with hands-on training from our very own esteemed cardiothoracic surgeon and exceptional educator, Dr. Darryl Weiman. ✅Why is this important? Because in most smaller hospitals, such enriching academic lectures often remain elusive. ✅What are we looking at? Positive pressure porcine lung models that were intubated so the lungs could be inflated to demonstrate effective sealing after the affected tissues had been removed. This also provided Dr Weiman with a specimen to show our team specialist cardiac techniques for open heart surgery. ✅What did we learn? This experience offered the academic environment so our team had time to absorb some valuable knowledge that Dr. Weimann graciously shared. Transcending beyond observation, our team also got some hands-on lab time to develop suture skills with an added opportunity to fire the surgical stapler on tissue, empowering them with valuable insights. This isn't just a lecture; it's a gateway to excellence, a beacon of opportunity, fueling our passion for innovation and advancing patient care. At the Banting Institute, cardiac surgeon Wilfred Bigelow and research fellow John Callaghan used hypothermia to slow the heart for open-heart surgery, discovering that cooling the heart below a certain temperature rendered it immobile. They struggled to induce cardiac contraction during hypothermia until Jack Hopps observed that electrical impulses could repetitively stimulate heart contractions. Hopps refined this process through experiments and, in 1950, designed a device at the NRC. Successfully testing it on a dog, which resembles a small table radio, and using vacuum tubes and household current, was a significant medical milestone. Hopps also developed transvenous catheter electrodes, allowing for heart stimulation without opening the chest.
