All too often, management makes the mistake of “guessing” what their people think or want… And ends up completely off base. Consistent surveys protect against this. Especially if they are anonymous… Asking > Assuming.

Here's what you need to know: Root Cause Focus: Look beyond the surface to uncover the root causes of incidents. This helps in taking targeted actions to eliminate or mitigate hazards. Fact-Finding, Not Fault-Finding: Incident investigations are about understanding what happened, not blaming individuals. Workers should feel comfortable speaking openly to investigators without fear of disciplinary action. Preventative Measures: Once the root causes are identified, take action to eliminate, isolate, or protect against hazards. Time Well Spent: Incident investigations are not a box-ticking exercise.
