Fri 2024-01-19 15:58 HEARTS in the Americas: Compendium of Essential Clinical Tools 2023 Since 2016, the Pan American Health Organization (PAHO) has been promoting the implementation of the HEARTS Initiative in the Americas as a regional adaptation of the World Health Organization's Global HEARTS Initiative. During this time, 33 countries in the Region of the Americas have committed to implementing HEARTS, and PAHO has developed many technical resources to support them. Most of these resources are clinical tools for primary healthcare teams and focus on quality improvement. This compendium aims to group all these clinical tools in a single document under a simple format that facilitates their practical implementation in daily clinical practice.

As researchers, one of the most important things we must focus on is translating good science into practice and policy. With Agency for Healthcare Research and Quality support we developed a tool to help accelerate the implementation of systematic approaches to diagnostic error measurement and learning across healthcare organizations. This enables them to identify opportunities to improve diagnostic safety practices. Our new paper, led by Andrea Bradford at Center for Innovations in Quality, Effectiveness, and Safety (IQuESt), evaluates the implementation of this resource to detect diagnostic errors for organizational learning and improvement. In Boston for the Society for Medical Decision Making 46th Annual Meeting? Join DPHS' Semra Ozdemir and her fellow panelists tomorrow, Oct. 29, at 2:35 p.m. for "The Power of Preferences: Shaping health decisions across diverse contexts." This session will introduce attendees to how preferences are defined, measured, and used to inform health and medical decision-making across diverse contexts, including shared decision-making, medical product development, health system design, policy, and health technology assessment. The session will also provide a novel framework for holistically considering preference information across diverse medical decision contexts. Janus’ Chief Medical Officer Dr. Paul Ciechanowski recently presented at the AHRQ Learning Collaborative, facilitated by Emilia De Marchis, MD, MAS, and Lyndsey Miller, PhD, RN. This series covering Person-Centered Care Planning for Persons with Multiple Chronic Conditions is such important work. Here are a few takeaways for those who were not able to attend this amazing event: 1.     The most compelling way that we can address the needs of persons with multiple chronic conditions is through a team-based, whole-person approach, considering bio-psycho-social factors. 2.     We know from years of research how much behavioral health factors into the overall outcomes and total cost of care in this highly vulnerable population. 3.     Collaborative Care addresses these psychosocial issues in patients with chronic conditions. Under Collaborative Care, “a team with a shared mission uses improved clinical systems to deliver improved care to a patient population supported by operational and financial systems. Such care is continuously evaluated through improvement processes and effectiveness measurement.” Thanks to the facilitators for giving Paul the opportunity to present, and a shout-out to Paul’s “colleague” Nate Flacker at ChenMed, who is also a member of the AHRQ Learning Collaborative, as our work together over the last year has been so rewarding. #personcenteredplannning, #multiplechronicconditions, #collaborativecare, #frombenchtopractice www.ahrq.gov
