I'm pleased this debate is going, but find myself unconvinced by this response to Kim Foster and John Hurley's editorial. 'Every nurse is a mental health nurse' means different things to different people. What the article seems to really say is "all nurses need to be able to respond to people with mental health problems with empathy and compassion", in which case, of course, everyone agrees. I think there is a philosophical difference between a 'mental health nurse' and 'general/adult nursing', in that there should be an underpinning ethos, knowledge and skillset which accounts for that empathy and compassion, yet contextualises this in the contested area of mental health where no-one agrees on eitiology or single established models of treatment, where there are huge power imbalances due to legislation which can remove human rights, and where there are huge ethical issues between autonomy and paternalism. There is a big difference between responding empathetically to someone with diabetes or cardiac issues, to a conversation with a man who believes that god wants him to cut off his own leg. Not every nurse is equipped to do that. Maybe there is a misunderstandinging between serious mental illness and what has now come to be called 'wellbeing'? The article makes some reasonable points, yet seems to miss many others. #MHDeservesBetter

As intended, our Editorial Kim Foster John Hurley 'every nurse is not a mental health nurse' has engendered further debate about our specialty profession. https://lnkd.in/gU9uxQaj Tony O'Brien's recent Editorial rebuts this with the contention that ‘every nurse is a mental health nurse’. As we know in mental health, language matters. How we speak about our field shapes and influences our own and others' perceptions and behaviors. We appreciate the arguments put forward by Tony O'Brien. But to be clear, our Editorial was not saying every nurse should not have mental health nursing knowledge and skills. They absolutely should. The question we were putting forward is whether this language about mental health nursing serves us. There is substantial evidence that our specialty field has been eroded, and our self-determination and autonomy affected, by the notion that ‘every nurse is a mental health nurse’. Some questions for our profession to consider: - Is ‘every nurse is a mental health nurse’ a valid position for us to hold? What are the implications of this for our specialty profession into the future? - How can ‘every nurse is a mental health nurse’ be used to undermine our autonomy as a specialty discipline? - What is our position on this debate? As an elected member of the Wisconsin Nurses Association’s Workforce Advocacy Council, it is crucial to hear from the nursing workforce in Wisconsin. I took my nomination to be an advocate on this very important work very seriously, and I’ve committed to addressing key items impacting our workforce shortage, especially mental health and well-being. If you have any insight or comments, please feel free to reach out to me personally. A few items elevated by nurses in Wisconsin and things being worked on: 1. Invasive questions about mental health on nursing licensure applications —UPDATE: in collaboration with Dr. Lorna Breen Heroes' Foundation, reviewing all initial and renewal licensure applications, *discovery of key stakeholders*, planned engagement with Department of Health Services partners. 2. Disclosure of nurse protected health information on state board proceedings as public record (mental health diagnosis, psychiatric evaluation results, alcohol and other drug abuse assessment (AODA) evaluation results, etc.). —UPDATE: engaging lived experience, utilizing storytelling to articulate the profound impact this has on nurses, especially the impact on the nursing workforce shortage, discovery of the disciplinary process, further discussion on next steps. 3. Wisconsin Nursing Peer Support —UPDATE: Involved with the creation of the WisPAN Wisconsin Peer Alliance for Nurses program in place; rejuvenation of the WNA’s Nurses Caring for Nurses program (seeking engaged WNA members to be involved - form is open until August 15th, 2024 - https://lnkd.in/g8hYP5kg)

🚨 New Article Alert 🚨 In my research, I explored the often overlooked but critical issue of moral distress in nursing—a negative psychological response arising when nurses cannot provide the ethical care they know is right due to external constraints. This challenge has been magnified by the COVID-19 pandemic, making it more urgent than ever to address. 🔍 Key Insights: Moral Distress and Burnout: Current nursing programs often struggle to adequately prepare students for the emotional and ethical challenges they will face. Nurses need more than just an awareness of resources; they need practical training in how to access and use them. Resiliency and Coping: Building resiliency through formal training in mindfulness, ethical competence, and self-care is crucial. Programs like the Resiliency Center and the Joy in Work Program at U of U Health can serve as models. Team Dynamics: Effective team dynamics significantly impact moral distress levels. Training in communication skills and team support can make a challenging shift manageable and improve patient care quality. Nurse-Patient Communication: Empathetic, honest communication helps manage moral distress. Phrases that validate and support patients can ease psychological strain for both nurses and patients. 📚 Next Steps: We must revise nursing curricula to integrate comprehensive training on moral distress, burnout, resiliency, and team dynamics. This approach will better prepare nurses to handle the complexities of healthcare with confidence and compassion. The urgency of addressing moral distress cannot be overstated. As the pandemic has shown, the resilience and well-being of our nurses are critical to the health of our communities. It's time for nursing education to reflect the realities of the profession and equip our nurses for all the challenges they will face.
