The article explores the complexities of managing transfusions for AIHA patients, particularly the challenges posed by serum auto-antibodies during pre-transfusion testing. I normally enjoy the challenge of crossmatching a new AIHA patient in the reference lab; getting the safest possible blood out in the quickest time. You do require the assistance of the NHSBT medic to confirm the right path to take but it is still down to the bench work to get the right result for the patient. From my limited experience in reference transfusion labs we rarely get samples that auto-adsorption would be possible due to transfusion history and amount of sample received. Ideally you would like to have the Duffy / Kidd genotype available along with the Rh / K type before issue of any units but not always possible due to time constraints. The majority of the patients we see will have been recently transfused so the phenotype might not be clear cut but there is normally enough evidence to make a solid guess. The protocols of allo-adsorption have changed in the different reference labs I have worked in. We use 4oC adsorption in some cases where there is a mixture of IgG / C3d involvement. In an ideal situation you could genotype all haematology cases as soon as they are identified to help with blood provision in the future.
