Medically fragile patients experiencing homelessness all too often receive either a lower level of care (I.e.-congregate shelter, street outreach, etc…) or a higher level of care (SNF, long term care, etc…) than what is needed to support their illness because of the dearth of available resources to fill the gap. We need more programs like these that fill care gaps and to think more broadly about the impact they have on the health and well-being of our communities.
The recently released evaluation of Project Roomkey showed what is possible when there is political will, cross-sector coordination and collaboration, and person-centered approaches to housing and health care. In this new California Health Care Foundation blog, you can read about two Alameda County Project Roomkey 'sheroes' - Alexis Chettiar and Catherine Hayes. These amazing clinicians leveraged their Roomkey success and learnings to create, in collaboration with Five Keys Schools and Programs and other key partners, transitional and permanent housing for medically frail people. 💥 It's about EACH Individual. 💥 I got a call to help a person who lives 5 hours away. They were diagnosed with a condition where UofL Health is leading the research and care in treating. The patient didn't know where to start. It was an honor to help this one patient. Much of my time focuses on system processes and developing new provider services. It can be easy to forget about purpose- care for each patient, one at a time. Healthcare has many complexities and challenges, but we can't lose sight of the fact that every individual has unique needs, fears, and hopes. It's our duty and privilege to provide them with compassionate, personalized care that honors their humanity. If you are a leader in healthcare, YOU KNOW the profound impact you can have on someone's life by treating them not just as a condition but as a whole person. Whether holding a hand, lending a listening ear, or going the extra mile to ensure comfort and healing, we strive to make a difference, one patient at a time. Remember, If a bald, (slightly) crazy COO can attempt to make an impact by leading with compassion and a laser focus on every patient, I can only imagine what you can do! Keep impacting the world!

Did you know that July is International self-care month? As per WHO, 24th June marks the start of Self-Care Month, which ends with Self-Care Day on 24th July. This symbolic day was chosen because self-care can be practiced “24 hours a day/7 days a week”. This year’s Self-Care Month marks the 5th anniversary of WHO launching the first global “Guideline on self-care interventions for health and well-being”. Self-Care Month is an opportunity to: * raise additional awareness about self-care and self-care interventions; * acknowledge the benefits that they bring to people’s lives and what’s been achieved so far; * call for renewed commitments and action to expand all health systems to include self-care interventions.
