🚫 Medication Review Cautions: -Be cautious with low eGFR or creatinine clearance (CrCl) when reviewing medications or handling requests. 💊 Specific Medication Guidelines: -If CrCl is <35 mL/min, avoid bisphosphonates as recommended by the BNF. Manufacturer advises avoiding metformin if eGFR is less than 30 mL/min/1.73 m². 🔍 DOACs Monitoring: -For Direct Oral Anticoagulants (DOACs), correct dosing and monitoring frequency are crucial. Ensure CrCl levels are checked. 🩺 Low eGFR Actions: -If a patient has a low eGFR, check if they are under nephrology care. -If not under nephrology, verify that drug doses are safe and appropriate. Refer to the BNF or the Renal Drug Handbook for guidance.

Daily Critical Care Pearls #13: Inotropes - Dobutamine 📌 The onset of action is 1 to 10 minutes and half-life elimination is 2 minutes 📌 The peak effect may not be seen until 10-20 minutes post-dose increase so watch for over-titration if it is initially done rapidly 📌 Titration may occur rapidly in increments of 2.5 mcg/kg/min every 5 minutes if there is a clinical need but 10 minutes may be safer 📌 Beta-blocker therapy, especially with carvedilol, may blunt the inotropic effects of dobutamine. This is not a contraindication so a decision may be made to use dobutamine over milrinone despite the interaction if milrinone is not indicated. #criticalcare #pharmacy #medicine #clinicalpharmacy #pearls

Daily Critical Care Pearls #28: Drug-Induced Ventricular Arrhythmias (VT) 📌  Medications that can cause monomorphic VT: Anesthetics (Bupivacaine, Ropivacaine), Antiarrhythmics (Adenosine, Amiodarone, etc.), Anticancer agents (5-Fluorouracil, Arsenic trioxide, Anthracyclines) 📌 Treatment of hemodynamically unstable monomorphic VT: urgent synchronized cardioversion,   Treatment of hemodynamically stable monomorphic VT: IV amiodarone, lidocaine, or procainamide and with synchronized cardioversion if necessary after appropriate sedation 📌 Lipid emulsion can be used in those with VT induced by bupivacaine and local anesthetics #criticalcare #pharmacy #medicine #clinicalpharmacy #pearls
