Improving Behavioral, Community and Tribal Health | Whole Person Care Technology Solutions | Believe in Better | Differentiator |
Community Health Centers: You're people caring for people. It's not just what you do, it's who you are, it's your mission. We understand care is everything. Access to care. High quality care. And we know it's not always easy. Regulations and reporting are complex and fluid, so having accurate data at fingertips is crucial. Rising costs and decline reimbursements challenge everyone, making it vital to have tools that bring in every dollar you earn, faster. Managing data from many of sources can be daunting, but our interoperability engine makes delivering coordinated, informed care easy. Systems should work the way you work, and not the other way around. They should flex to you. Staying fully staffed and avoiding burnout is a real issue. Our intelligent automation running in the background takes mundane tasks off your staff so they can focus on higher value jobs. For providers, we're making it easier for you to better care for and truly connect with your patients. With intuitive, simple workflows. Charting accelerators. All your preferences. And AI that listens to your conversation and creates the bulk of the encounter note for you within seconds. It's doing the heavy lifting for you so you can be entirely patient-centered. And in today's always on world, people expect healthcare on their terms, they want answers yesterday. Our patient engagement tools keep your patients at the center of all you do. This goes beyond keeping them engaged, it's about guiding them to success. Because when your patients win, you do too! That's why you're doing this meaningful work in the first place. We've created an environment that maximizes outcomes. By design, it functions to help you deliver better healthcare for everyone. For your patients and clients, their families, and the communities at large that you serve. National Association of Community Health Centers (NACHC) #believeinbetter #communityhealthcenters #fqhc

WHAT IS KNOWN Coalitions such as Accountable Communities for Health (ACHs) can foster new or more diverse partnerships and increase alignment between organizations and across sectors. WHAT THIS STUDY ADDS ACHs in California spurred new organizational connections, strengthened organizations' ability to work together, and increased focus on community engagement and equity but made less progress on community ownership. CONCLUSION The ACH model is effective at strengthening organizational partnerships and catalyzing other systems changes and precursors including enhanced knowledge, increased capacity, more collaborative ways of working, and new funding.
