Our new paper, which assessed the cost-effectiveness of the disease-management program "HerzMobil Tirol" for patients with heart failure, is now online and open access! 📯 Key insight: Our findings show that adopting telemedicine-assisted programs for heart failure is cost-effective comapred to usual care! Congratulations to Dr. Sukesh Burjonroppa, Dr. Richard Vigness and the entire Texas Health Fort Worth Texas Health Resources structural heart team for their recent success in completing the first BASILICA procedure in Tarrant County! What better time than "heart month" to mark this significant milestone. This breakthrough procedure provides a new treatment option for patients who have higher risk for coronary artery obstruction during Transcatheter Aortic Valve Replacement (TAVR) relating to their unique anatomy. When someone undergoes TAVR evaluation, scans may show the existing aortic valve has especially long leaflets - the flaps that open and close with each heartbeat; or the coronary arteries may sit unusually close to the aortic valve. During TAVR, the replacement valve may then push the existing leaflets into the coronary arteries causing a blockage - a rare but potentially fatal complication. But with BASILICA (Bioprosthetic Aortic Scallop Intentional Laceration to Prevent Iatrogenic Coronary Artery Obstruction during TAVR), doctors use a tiny electrified wire to slice and push aside existing valve flaps, thereby avoiding the risk of coronary artery obstruction. "This is a niche procedure for a small subset of our patient population," explains Dr. Burjonroppa. "But there would be significant obstruction if that leaflet was not removed. By doing this, we make sure there is an adequate opening for blood flow. For this particular patient it was a great procedure. She's feeling like a million bucks - no more shortness of breath! Andrew Holden (Auckland, New Zealand), one of the co-chairs of the Charing Cross (CX) International Symposium, presented the final five-year data on the vital status of patients enrolled in the IN.PACT AV Access pivotal trial in April 2024 as a podium-first presentation. Due to the paclitaxel safety concerns raised, the researchers re-consented patients enrolled in the trial to prolong the follow-up period to establish their long-term vital status. “The all-cause mortality remains the same out to five years. There is no mortality concern using the IN.PACT [AV] drug-coated balloon (DCB). The long-term durability of this study really shows that the IN.PACT DCB should be the standard of care in maintaining [the] AV [arteriovenous] access circuit,” Holden said, reminding the audience that this is the only randomised, pivotal trial in AV access circuits to demonstrate a consistent and sustained clinical benefit out to three years. The IN.PACT AV Access randomised controlled trial comparing plain balloon angioplasty with DCB angioplasty in managing stenotic disease in AV access has previously shown significantly improved sustained benefits for end-stage kidney disease patients at the three-year time point. Further research has also demonstrated a cost-effectiveness advantage for healthcare systems in Japan, Korea and the USA, when the device is used.
