On 17 November 2021, Lockheed Martin F-35B Lightning II ZM152 (BK-18) of Royal Air Force (RAF) 617 Squadron, crashed into the eastern Mediterranean, during take off from HMS Queen Elizabeth. As the ship sailed past the wreckage before it sank, an intake blank was seen to float free. Was the loss of this £181.1 million fighter simply because ‘someone’ was negligent or incompetent? No. This deep dive case study demonstrates that real life is more complex. #aviation #aviationsafety #flightdeck #humanfactors #hmi #CIEHF #RAeS
