Great to get the systematic review protocol I worked on with Dr. Nuala Ryan, Briga Hynes and @Aisling Finnucane published in the BMJ Open. Proud to be a part of this recent publication in BMJ:Q&S with Myrtede Alfred, Laura Barg-Walkow, and Alex Chaparro. If you aren't sure whether you need to use a checklist or not, or don't know what type to use, this paper reviews an algorithm for making these decisions. From our abstract: "Checklists are a type of cognitive aid used to guide task performance; they have been adopted as an important safety intervention throughout many high-risk industries. They have become an ubiquitous tool in many medical settings due to being easily accessible and perceived as easy to design and implement. However, there is a lack of understanding for when to use checklists and how to design them, leading to substandard use and suboptimal effectiveness of this intervention in medical settings. The design of a checklist must consider many factors including what types of errors it is intended to address, the experience and technical competencies of the targeted users, and the specific tools or equipment that will be used. Although several taxonomies have been proposed for classifying checklist types, there is, however, little guidance on selecting the most appropriate checklist type, nor how differences in user expertise can influence the design of the checklist. 𝗗𝗶𝗱 𝘆𝗼𝘂 𝗸𝗻𝗼𝘄 𝘁𝗵𝗮𝘁 𝘁𝗵𝗲 "𝗵𝗮𝗹𝗳-𝗹𝗶𝗳𝗲" 𝗼𝗳 𝗮 𝗽𝗿𝗶𝗻𝘁𝗲𝗱 𝗽𝗮𝘁𝗶𝗲𝗻𝘁 𝗹𝗶𝘀𝘁 𝗶𝗻 𝗮 𝗵𝗼𝘀𝗽𝗶𝘁𝗮𝗹 𝗶𝘀 𝗷𝘂𝘀𝘁 𝟯.𝟯 𝗵𝗼𝘂𝗿𝘀 (https://lnkd.in/g36v8m8C) Think about that. Within a few short hours, the paper lists we rely on for patient care—patient lists, rounding lists, handoff lists—are outdated, yet we continue to use them for the rest of the day (on average, for 9-12 hours). This is not just a minor inconvenience; 𝘪𝘵’𝘴 𝘢 𝘵𝘪𝘤𝘬𝘪𝘯𝘨 𝘵𝘪𝘮𝘦 𝘣𝘰𝘮𝘣. Unfortunately most of the errors caused by this workflow paradigm are not recognized, and fly under the radar. #PatientSafety 𝙸̲𝚖̲𝚊̲𝚐̲𝚒̲𝚗̲𝚎̲ ̲𝚝̲𝚑̲𝚒̲𝚜̲:̲ An intern starts their day with a printed list that’s accurate at 7 AM. They start rounds with their team. By 11am, when they are discussing a patient with a GI bleed, they are using the Hg printed on their list from 5am - which was normal - instead of seeing the new Hg from 10:30am which is not yet “critical” but shows a significant drop. The plan they set for the day is now based on the old Hg.  Now they will most likely see the updated Hg once they are back at the computer, but that might still be an hour a way. Maybe more. That means the best plan for this patient has been delayed. These kinds of errors have a butterfly effect and a cause grave errors that no one realizes are from this initial issue of making a decision off of a couple hour old lab value. #PreventableMedicalErrors 𝗜𝗻 𝗮 𝘄𝗼𝗿𝗹𝗱 𝘄𝗵𝗲𝗿𝗲 𝗰𝗹𝗶𝗻𝗶𝗰𝗮𝗹 𝗱𝗲𝗰𝗶𝘀𝗶𝗼𝗻𝘀 𝗺𝘂𝘀𝘁 𝗯𝗲 𝗺𝗮𝗱𝗲 𝘀𝘄𝗶𝗳𝘁𝗹𝘆 𝗮𝗻𝗱 𝗮𝗰𝗰𝘂𝗿𝗮𝘁𝗲𝗹𝘆, 𝗿𝗲𝗹𝘆𝗶𝗻𝗴 𝗼𝗻 𝗼𝘂𝘁𝗱𝗮𝘁𝗲𝗱 𝗶𝗻𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻 𝗶𝘀 𝗹𝗶𝗸𝗲 𝗻𝗮𝘃𝗶𝗴𝗮𝘁𝗶𝗻𝗴 𝘄𝗶𝘁𝗵 𝗮 𝗺𝗮𝗽 𝘁𝗵𝗮𝘁 𝗵𝗮𝘀𝗻’𝘁 𝗯𝗲𝗲𝗻 𝘂𝗽𝗱𝗮𝘁𝗲𝗱 𝗶𝗻 𝘆𝗲𝗮𝗿𝘀. We wouldn’t trust an old map to guide us through new terrain, so why do we trust old data to guide patient care? It’s time to phase out paper and embrace real-time, well-designed mobile tools that provide the accuracy and accessibility clinicians need to make informed decisions. !!!New publication!!! We explored General Practice (GP) recording of carer status and the number of patients self-identifying as carers, while self-completing an automated check-in screen prior to a GP consultation among the 11 participating GPs in the West Midlands, England. 80.3% (n=9301) of patients self-completing an automated check-in screen participated in QUantifying the identification Of carers in general practice (STATUS QUO Study) (62.6% (n=5822) female, mean age 52.9 years (10–98 years, SD=20.3)). Prior to recruitment, the clinical code used to denote a carer was identified in 2.7% (n=2739) of medical records across the participating GPs. 10.1% (n=936) of participants identified themselves as a carer. They reported feeling supported with their own health and social care needs: always 19.3% (n=150), a lot of the time 13.2% (n=102), some of the time 40.8% (n=317) and never 26.7% (n=207). Many more participants self-identified as a carer than were recorded on participating GP lists. Improvements in the recording of the population’s caring status need to be actioned, to ensure that supportive implementation strategies for carers are effectively received. https://lnkd.in/ew6C7Tzt

"Creating an enabling environment for organisational learning is a strategic design decision. Enabling conditions for a learning health system have been described as a combination of leadership and culture, system design and necessary financial and human resources." Ecosystems like global health don't just advance by smart people doing smart things in their silos. Building shared consciousness across the ecosystem that sifts out what's not helpful and distills what is, that's what moves things forward. And that doesn't happen unless the orgs in that ecosystem commit to being part of that effort, set aside their organizational egos and design their Operating Systems in service of that larger collective effort. Big props to Breanna Wodnik for this call to action and challenge to the leaders of major orgs in global health to put their money where their mouth is as far as ecosystem-level learning is concerned. (And...I can attest, having Breanna as a condo neighbor made for great conversations over the fire pit and while walking dogs around the neighborhood...been a great ride!)
