How are you breaking YOUR mold? I think that's the challenge every day - to not just counter others' preconceptions, but our own. When I lead workshops on using the principles of improv in medical practice and life (a recent participant said, "You just re-taught me everything I need to know about being a parent" in an improv for healthcare session!), I challenge participants to break the type-casting they've been doing for themselves. You know... What it needs to look like to be a great doctor, a good mom, a supportive partner, a giving community member... things that we just strive for and often find ourselves falling short. Because we weren't meant to play those roles. "Nobody wants to see the perfect nun," - my mentor guiding me in a not-so-hilarious improv rehearsal. "Let's see their imperfections - THAT's what makes an interesting character and human. It's the human part." Game on. For most of my chaplaincy career, I would often colloquially refer to my clinical note as my spiritual assessment (clinical note = spiritual assessment). With my PhD work concerning the #palliativecare chaplain’s clinical note, I have discontinued this conversational reference. The #spiritualassessment is arguably the most important part of a chaplain's note. But it is just that, a part. #Chaplain clinical notes are also made up of other aspects contributing to the holistic care for patients and their care recipients. Fellow physicians, what’s stopping us from taking enough time off- whatever that may be and realizing it’s different for each of us? Just the other day, I had a conversation with a GI doc who revealed he’s only taken 3 weeks off per year for the last 20 years. He’s thinking about, and really wants to take 12 weeks off this year. My take- do it! You only live once. Life is short and all of those cliches. After completing the Certification of Clinical Vestibular Competency at Johns Hopkins University School of Medicine last year, my assessment and treatment abilities are much more extensive, as are my skills and knowledge. I've tried to absorb increases in overhead costs for many years, not changing my rates since January 2018. But the time has come to raise my prices. Effective June 1, 2024, the prices for any new patients will be subject to increased per-visit rates. The new rates for new patients will be posted at BetterBalanceInLife.com after June 1. Family Medicine - How to Reward Comprehensiveness? The FHO is a great model, but has flaws. There should be some reward for doctors who are comprehensive and manage complexity 'in house.' As opposed to "Referologists" who have huge rosters but simply refer everything out to Specialists. The challenge is: if there was a "Referral Reduction Bonus", that would harm patients. Because surely doctors would inappropriately NOT refer when referrals are truly needed. Any ideas how to reconcile this concept?
