Case update. Facial space swelling of Lt Submandibular region Akhtar Bibi 54Y/F, know D.M (10y) and HTN(on off medication) presented with history of pain and swelling since 1 week. Previously patient was advised I and D but patient refused and was taking IV medication. O/E: swelling on left of face involving Lt Buccal and sub mandibular region which tender to touch. Mouth opening is 8mm Poor oral hygiene Aspiration+ve( advised for Pus C/S) I&D done, draine placed at Left Retro molar area secured with Slik 3/0. Drain is active BLIS done OPG misplaced ( they will trace from IDC) HB: 13.2 g/dl WBC: 10920/uL P: 344k HbA1c:15.3% BSR( 216, 247, 382,250g/dl) Treatment plan: G.M consultation done, they advised R insulin 120units TDS BSR and BP charting continue Medication sheet maintained Patient retained in ER under OMFS care

An infusion pump infuses fluids, medication or nutrients into a patient's circulatory system. It is generally used intravenously, although subcutaneous, arterial and epidural infusions are occasionally used. A type of infusion pump, manufactured by Fresenius. Infusion pumps can administer fluids in ways that would be impractically expensive or unreliable if performed manually by nursing staff. For example, they can administer as little as 0.1 mL per hour injections (too small for a drip), injections every minute, injections with repeated boluses requested by the patient, up to maximum number per hour (e.g. in patient-controlled analgesia), or fluids whose volumes vary by the time of day. #snsinstitutions #snsdesignthinkers #designthinking

Proper administration of CPR MEDICATIONS via an ENDOTRACHEAL TUBE Interrupt chest compressions (which you should be avoiding if at all possible) & inject a mixture of the medication in 10mL Normal saline or distilled water into the endotracheal tube, w/ several subsequent bag−valve mask ventilations prior to resuming chest compressions. Decoding “DICKS”: Managing Hyperkalemia (elevated blood potassium levels) In healthcare, acronyms can be quite cheeky, like “DICKS.” Before you think otherwise, this one is all about tackling hyperkalemia, a medical emergency demanding immediate attention. DICKS breaks down like this: *D - 50% Dextrose IV push *I - Regular Insulin IV push *C - Calcium Gluconate IV *K - Kayexalate orally or rectally *S - Sodium Bicarbonate IV (if patient is acidotic) These interventions, in accordance with facility protocols and physician preferences, are deployed to swiftly bring the levels back to normal. (However: It is recommended to administer Calcium Gluconate first, due to its “heart-protective” effects.) ⛔ IS INTUBATION THROUGH A LMA OVER A FIBREOPTIC SCOPE(FOB) EASIER AND SAFER THAN USING A LARYNGOSCOPE FOR PRACTITIONERS THAT INFREQUENTLY INTUBATE? ⛔ 1️⃣ the patient will be less likely to suffer hypoxia as ventilation can be continued even while intubating 2️⃣ if intubation fails the Laryngeal Mask Airway can always be used to ventilate the patient 3️⃣ if the endotracheal tube is advanced just past the elevating bars in the distal LMA tube there is only 2.5 to 5cm to negotiate between the tube tip and the vocal cords with the FOB and the scope should be orientated towards the vocal cords in most attempts. 4️⃣ placing a Laryngeal Mask Airway is simpler than intubation, requires less anaesthesia and only if ventilation is achieved does the patient need to be paralysed. ✅ Using a FOB through a Laryngeal Mask Airway is an easy skill to master. ✅ Certainly for infrequent practitioners intubating ill and compromised patients via the LMA has the potential to reduce intubation related complications and should be considered before using a laryngoscope. hashtag #innovation hashtag #medicaldevices hashtag #paramedics hashtag #emergencydepartment hashtag #emergencymedicine hashtag #prehospitalcare hashtag #airambulance the 🔺 DA-ETT 🔺 is available from Lateral Medical, Brisbane www.lateralmedical.com
