Tiday’s case : Failed Hancock # 23 bioprosthetic valve ( non fracturable) . Valve in Valve TAVI done today using an Evolut FX 23 mm . STJ 37x33 , VTRCA 7 mm, VTLMCA 14 mm Severe tortuousity of the iliofemorals Gore dry seal sheath delivered over Supra core wire - straightened the vessel beautifully. Valve in valve performed over lunderquist wire Great positioning and deployment of the Evolut with post dilatation using an 18 mm True balloon. Excellent final result with minimal gradient. Take home points : Handle iliofemoral tortuousity seriously as it may be a good enough reason to ruin your primary access site. Safari vs Lunderquist in valve in valve cases - we have seen the lunderquist to provide more stability and valve stabilization at the outer curvature Fracturable / unfracturable bioprostehtic valves and balloon sizing accordingly.
