The central approach to the internal jugular vein (IJV) is commonly used for central venous line (CVL) insertion because it may reduce the risk of pleural or carotid arterial puncture. However, multiple studies have found that catheters in the IJV have a higher rate of colonization or infection compared to those in the subclavian vein. Proper positioning of the jugular line is crucial, as it is often contaminated by saliva and secretions. This depends on the physician's skill and knowledge. Effective collaboration between nurses and doctors, along with adequate training and competency for physicians, is essential to ensure correct positioning and greatly reduce the risk of infection.

Let's talk about this! We have decades of work describing and measuring the contribution of nurses in the hospital setting. The industry has agreed upon major nursing sensitive indicators: HAPI, CAUTI, CLABSI, VAE, Falls with Injury. These measurements are intended to quantify the nurse's influence on quality care. What is your organization measuring in ambulatory care to describe and quantify the nurse's influence on quality care? Have you talked to your nurses about whether they feel like they have a direct influence on that metric? Are they using those metrics to drive their improvement projects? Do they even know this is the way their performance is being measured? We have work to do. What is the ambulatory care equivalent to CAUTIs and CLABSIs??? I now work in pediatric specialties: it's not screening for breast or colon CA; it's not HTN screening and follow up.
