An estimated 321,566 children in the United States lost a parent to drug overdose from 2011 to 2021, according to a study published in JAMA Psychiatry. The rate of children who experienced this loss more than doubled during this period, from approximately 27 to 63 children per 100,000. The highest number of affected children were those with non-Hispanic white parents, but communities of color and tribal communities were disproportionately affected. Who owns the journey? Part 3 When I left the NHS in 2014, after 25 years – 15 of them had been in developing the specialty of substance misuse treatment in a systemic approach with young people. It’s a whole branch of the journey that on leaving, took me into along a very different path into developing a therapeutic approach and innovation in the field of residential care for children and young people. Even when I’d been presenting the work of specialist substance misuse teams to fellow child psychiatrists, I was asked, “aren’t you doing the job of a social worker?”. I remember answering that question at a conference with over 100 Psychiatrists in the audience, saying - I take that as a compliment - this is Social Psychiatry - this is seeing the individual differently and not in a medical model or diagnostic box but in a social context. Birribi was formed in 2014 – initially with the aim of setting it up as a social enterprise company and trying to get interest in the potential of grant funding from social business Wales. At the time and during the first three years of setting up, people just weren’t thinking about social innovation work with disadvantaged youth at the centre and there was no backing. So, the vision and innovation and research development through Birribi, has only ever been possible through investing our own personal funds into a limited company. In shaping the work of Birribi we hold true to the concepts of a multisystemic approach and relational practice at the core – with an overarching framework of therapeutic community practice. Surely that makes sense – a social learning and relational model for working with children, young people and families who are caught up in the social care system. The landscape of this field of work has obviously changed considerably over the last 10 years and there is such turbulence in the sector at the moment that we risk losing the grounded principles of working ethically with children and young people have suffered abuse, neglect and harm – often from early childhood. We have a overarching framework and ethos of working as a therapeutic community, so that when children join a residential community they’re supported through relationships with adult carers, and through building positive relationships with their families friends and developing social networks. It’s hard, but so important, to retain that ethos that every individual should have ownership of the narrative of their journey even when their journeys have been fraught with the most severe levels of adversity and trauma. #ACEs #therapeuticcommunities #systemicpractice #socialpsychiatry #equineassistedtherapy

This is a fascinating new study published in the journal JAMA Psychiatry: Adults who moved frequently in childhood have significantly more risk of suffering from #depression than their counterparts who stayed put in a community. We know that “adverse childhood experiences” can have a lasting impact on children as they journey through their teens and into adulthood. And whilst the results are limited geographically, what this points to is how great an impact disruption and instability can have on young children. Here are some of the points that really stood out to me: ▶ The risk of moving frequently in childhood was significantly greater than the risk of living in a poor neighbourhood ▶ Adults who moved more than once between the ages of 10 and 15 were 61% more likely to suffer from depression in adulthood compared with counterparts who had not moved (even after controlling for a range of other individual-level factors) ▶ The negative impact of a move was not mitigated by moving to a more affluent area: adults who had moved from the poorest quintile of neighbourhoods to the richest quintile had a 13% higher risk than counterparts who did not move. Those who moved from the richest to the poorest, by comparison, had an 18% higher risk than counterparts who did not move ▶ The paper’s lead author, professor Clive Sabel explains that one clear policy application is for the management of children in state care - for this vulnerable group, frequent moves between homes should be avoided.
