Thank you very much for sharing Karen; this is truly impressive and inspiring! It confirms my impression of UMass during my Grand Rounds two months ago. Remote patient monitoring is on the rise. Why? Because so many patients with rheumatic symptoms have to wait far too long for in-person consultations, leading to delayed diagnoses and missed disease flares. With new biomarkers and multimodal monitoring, we can achieve much better tracking and diagnostics. Of course, a traditional consultation with a clinical examination and ultrasound is irreplaceable. On the other hand, we sometimes see certain patients too often or even unnecessarily.
It’s also clear that the mental aspect and counseling should not be underestimated. However, there are groups of patients who prefer a fast and efficient remote consultation, for example, because they live far away or desire effective treatment and are capable of managing their condition independently. Why don’t we already have such virtual care centers in many other clinics in Europe? Because such a transformation requires considering numerous other factors, including reimbursement, as well as extensive change management, for instance, in communication with the clinical team and nursing staff within the clinic.
The video clearly demonstrates that such a centralized remote care center requires substantial investment, from the facilities to the training of staff and even patients themselves. However, even in Switzerland, cost pressures are increasing, and a shift in mindset is necessary. I’m impressed that patient management here is conducted very horizontally, even extending to monitoring intensive care units.
This raises the question for me: Who will take over remote patient monitoring in rheumatology in the future? For example, who will oversee the monitoring of patients with pre-rheumatoid arthritis? Such services outside clinics are becoming more prevalent. There is much we can learn from other disciplines in this area.
