American movie lovers watching the Oscars telecast were given a bit of super-techno, sci-fi as the AUKUS nuclear-powered submarine pact was advertised between award categories. The expensive-looking, prime-time advertisement, designed to boost the US naval industrial base, has the feel of a Hollywood movie preview with its James Bond-like submarine bay, dramatic music and basso profundo voice-over. I am not sure its genesis but it looks like a bit of pro-Biden political marketing with its combination focus on military allegiance and defence jobs. What works… Coming from the world of aerospace and defense, then advanced technology consulting, and now commercial real estate, the phrase I personally can’t stand is “this is the way we have always done it”. “Issues in communication and management cause projects to fail catastrophically”. What’s that? That’s the Vasa Syndrome. The Swedish warship “Vasa” sank on its maiden voyage in 1628. Gustavus Adolphus, King of Sweden, constantly meddled with the design despite having no expertise in shipbuilding, and various parties involved failed to #communicate effectively, partly out of fear of incurring the wrath of Adolphus. 7 core issues that comprise #VasaSyndrome:  1: Lack of external #learning capability 2: Goal confusion 3: Obsession with speed 4: #Feedback system failure 5: Communication barriers 6: Poor #organizational memory 7: Top-management meddling Despite no specific mention to #psychologicalsafety (remember, the paper was published in 2001*), we have no doubt that a lack of both psychological and probably physical #safety was a key causal component of this disaster.
