I am excited to announce that I have achieved a USMLE Step 2 score of 268 points! 🎉 The USMLE (United States Medical Licensing Examination) is an essential exam for medical professionals seeking licensure in the United States. Scoring well on this test demonstrates a high level of knowledge and competency, and I am incredibly proud of my achievement. This accomplishment would not have been possible without hard work, dedication, and support from my mentors, colleagues, friends, and family. The preparation process was rigorous but rewarding as it allowed me to deepen my understanding of crucial clinical concepts. As part of this journey towards becoming a licensed physician in the U.S., Step 2 evaluates practical application by assessing one's ability to diagnose health conditions accurately. This section includes topics like internal medicine, surgery obstetrics & gynecology pediatrics psychiatry epidemiology biostatistics, etc. It is important to note that while achieving an excellent score reflects proficiency within these subject areas; however, it does not solely define one's capability or expertise as a healthcare professional. Practical experience gained through rotations and working alongside seasoned physicians plays an equally vital role in shaping our careers as doctors. To aspiring medical students preparing for their own exams let me tell you - perseverance pays off! For international medical graduates (IMGs) aiming to match into a residency program in the United States through the USMLE, including clinical rotations in your resume is essential. While there isn't a strict number of rotations required, having a diverse range of clinical experiences can enhance your competitiveness. Aim for a minimum of 3 to 4 clinical rotations in core specialties such as internal medicine, surgery, pediatrics, and obstetrics/gynecology. Additionally, consider rotations in subspecialties or areas of interest relevant to your chosen specialty. Highlighting your clinical rotations in your resume demonstrates your hands-on experience, exposure to the U.S. healthcare system, and adaptability to different medical settings. Remember to include details such as the duration of each rotation, the name of the institution/hospital, the supervising physician, and any notable achievements or responsibilities during the rotation. Ultimately, the quality and depth of your clinical experiences, along with strong USMLE scores and supporting documentation, will contribute to your success in the residency match process as an IMG. #usmle #img #observership

🧠 Cognitive Biases in Pediatric Cardiac Care: When Our Cognitive Systems Are Responsible for Most Medical Errors 💔 ➡ Our recently published mini-review in Frontiers in Cardiovascular Medicine highlights how we are all prone to systematic reasoning errors 👀. Discover how these biases can influence our medical decisions: - Why will an ED colleague call you more often for benign murmur assessments if they have previously missed a congenital heart disease diagnosis in their practice? - Why is it so difficult to step back from a first impression during the decision-making process? - Why do you change your assessment (even when you were right) of a situation when some of your peers seem to oppose your decision in staff meetings? - Why is it so difficult to take into account a parent's complaint when they have already consulted four times without conclusive results? These insights into cognitive systems are not new concepts, and I do not claim to be an expert! Here are a few nice stats from Match Day 2024: Approximately 75 percent of the graduating medical school class will remain in New York State for their residencies. A total of 148 graduates (73.6 percent of those who matched) will remain in New York State. There are 103 graduates (51 percent) who will remain in New York City, including 31 who will become residents at Downstate. Consequently, more Downstate graduates practice medicine in New York City and New York State than from any other U.S. medical school. Top specialties among the Class of 2024 include Medicine, 18 percent; Anesthesiology, 12 percent; Psychiatry, 8 percent; Emergency Medicine, 8 percent; Pediatrics, 7 percent; General Surgery, 5 percent; and Urology, 5 percent. The Class of 2024 will pursue residencies in 29 different specialties. The University of Health Sciences (UHS) has officially endorsed changes to the exam pattern for the intermediate module exam (IMM) within MS and MD postgraduate clinical degree programs. The approval, granted during the 196th meeting of the Advanced Studies and Research Board (ASRB), led by Vice-Chancellor Prof Ahsan Waheed Rathore, introduces a unified approach. Instead of separate papers for specializations, a single paper for General Surgery and another for General Medicine will each contain 100 multiple-choice questions. Notably, specialty-related questions are removed from IMM. The Surgery paper, excluding Anesthesia, will focus on principles and basic sciences, while the Anesthesia paper will integrate questions from general medicine, general surgery, and basic sciences. Excited, Intrigued, Grateful, Inspired, Encouraged, Thankful. These are many of the feelings I had working with Alvaro Moreira this past weekend at the 2024 Neonatal-Perinatal Practice Strategies of the American Academy of Pediatrics in Scottsdale #WNPPS2024. #AAPneonatal Dr. Moreira is a role model clinical educator, data scientist, and innovator in neonatal critical care medicine. He provided an update on his current studies combining omics and #AI to help us better understand potential etiologies and mechanisms underlying the significant pathology of BPD; a disease related to prematurity and many other factors including biotrauma, volutrauma, barotrauma, atelectotrauma, oxygen toxicity, inflammation among many others. He is a dedicated investigator, role model, mentor, and just an amazing person with an incredible healthcare journey. He is one to watch for as are many of the investigators that are part of NeoMINDAI. For each patient he studied 32,000+ genes. Wow! incredible work. He kindly invited me to share the podium with him during some break-out sessions--- focused on how neonatologists and other clinicians could be using #LLM in their daily work lives. We highlighted how publicly available LLMs can be used to: brainstorm, refine writing, evaluate scientific (and publicly accessible) pdf articles, search medical literature more efficiently, analyze images to provide insight (ventilator graphics, pdf article graphics, pulmonary function test...), create educational slides, questions, simulations, and videos among many others. The sessions were very well attended. As in most #healthcare settings, we found that half of the audience had used a LLM, with only a handful using one the previous week. It seems that only early adopters in neonatology have started using LLMs and quite variably. We were at first met with significant skepticism 🤔 by some in the audience, but many, after listening and interacting with us during our presentations, agreed that it was time for clinicians 😷 to be more involved in understanding the possible applications of #AI in healthcare to ensure it is studied well, safe, effective, ethical, and unbiased when used in our NICUs. Our message: -AI, and in particular, LLMs are here. -They are not going away. -We need to leverage their capabilities as tools to make our daily work more efficient and effective. -We, as clinicians, need to be involved in the evaluation, implementation, & application in medicine. When was the last time you used a piece of paper and a pencil 📝 to do a mathematical calculation? How about a calculator or computer? LLM will soon be viewed much as the same way a computer or calculator is -- just a tool 🛠 we use for certain tasks. LLMs are not ready for primetime in clinical care, but certainly are superior for some tasks we highlighted. Prior to starting my obstetrics and gynaecology rotation, I found it necessary to review some theory, as it had been almost three years since I last studied OBGYN during medical school. To facilitate this process, I turned to Scrintal, a digital canvas designed to convert creative ideas into structured knowledge.   During my time in medical school, I relied on using flashcards to prepare for my exams. However, I encountered limitations with this approach, as it primarily aids in memorizing individual facts without facilitating the connection of different ideas to understand and remember concepts in context as a whole.   Scrintal, on the other hand, strikes the perfect balance between creative chaos and structured clarity. It allows users to brainstorm and refine their ideas, akin to utilizing a whiteboard. This visual note-taking system enables the capture of creative ideas using a canvas, boards, and cards, thereby visually linking ideas to structure thoughts.   Personally, I appreciate how Scrintal enables me to thoroughly scope a subject, visually annotate my notes, and effectively test myself for future retention.
