#futurefriday: on #wellbeing during #residency "The intensity of #training is associated with significant reduction in well-being markers in #residents, including sleep, exercise, family interactions, religious activity, and an increase in missing significant events." In this systematic review by Kristin Raj (J Grad Med Educ. 2016 Dec; 8(5): 674–684.) factors associated with well-being 😃 were identified in the literature. Some of the results (presented in Table 2): - Resident well-being overall was lower than population well-being! - More residents reported a worse #worklifebalance than faculty, indicated more work-life conflicts, and reported they felt too tired or irritable after work to participate in home life 😤 - Autonomy, competence, and social relatedness were the areas found to be widely associated with greater resident well-being - A greater amount of sleep, physical activity, and time in nature were associated with measures of greater well-being or less burnout 😴 - Time away from work and being in a relationship are associated with greater well-being 🤗 --> Being a woman, under age 32, and Caucasian were each associated with not using a resident mental well-being program because of feeling unable to take time for an appointment... Conclusions: -->> The major factors associated with resident well-being are #autonomy, building of #competence, strong social #relatedness, #sleep, and #timeaway from work. -->> #Perseverance is predictive of well-being, and greater wellbeing is associated with increased resident empathy. “Chronically forcing people to suboptimally perform” - a note from Douglas Lindsay, PhD on the current state of institutionalized #wellness deprivation… a big idea I know. But it’s true! Think of schools, which wake people up in knowing misalignment with circadian rhythms: “The American Academy of Pediatrics and the CDC both recommend school start times begin no earlier than 8:30 a.m. Schools are not listening. The average school start time in the United States is 8:03 a.m.” How about teaching health curricula that advises against certain foods, then serves those same foods in the lunchroom, or stocks vending machines with the sort of #Nutrition that contributes to preventable diseases? How about prolonged sedentariness? Research suggests it is both physically, psychologically, and cognitively degrading - but that’s how most have designed their days. In the fast-paced and emotionally charged environment of hospital work, caregivers, often nurses, doctors, and support staff, dedicate themselves tirelessly to the well-being of their patients. Despite the demanding nature of their work, caregivers frequently prioritize the needs of their patients above their own. However, self-care is crucial for providing compassionate and effective care. Mid-life crisis, (noun): a loss of self-confidence and feeling of anxiety or disappointment that can occur in early middle age. Now recent research suggests that there may be significant long-term societal health dangers lurking from this common syndrome. Drs. Sheehan and Infurna from the University of Arizona studied the sleep patterns in different generations of around 200,000 Americans. They found that things are getting worse, indeed much worse. While 25% of adults born a century ago were already struggling to get enough sleep during their midlife years, the next generation, born in the 40’s, the number of chronically sleep-deprived mid-lifers had grown to nearly 40%! To paraphrase the authors: “(this) pronounced decline in healthy sleeping patterns in the current generation of midlife adults is likely to have significant implications for the well-being and longevity of the current generation of midlife adults as well as the generations following in their footsteps. Policymakers, clinicians, and researchers should recognize the ‘canary in a coal mine’ and find ways to improve the sleep quality of midlife adults." They point to three causes that should be addressed through improved public policies and educational programs as well as individual healthcare programs. Specificall they are the widespread use of smartphones, increased occupational and socioeconomic instability, and changes in caregiving responsibilities. We know that 80% of adults that suffer from sleep disturbed breathing that impacts directly on sleep quality are unaware of the dangers. Recognizing this, our team @breathsimple has developed simple, low-cost methods to create awareness by screeing for problems before they become dangerous and to recommend appropriate treatments. 📣 The effect and implementation evaluation of the PerfectFit@Night intervention got published! ⚒ Four years ago, we developed a multi-faceted intervention to improve sleep, fatigue and need for recovery of night shift workers in healthcare. 🏨 PerfectFit@Night was implemented at 12 departments of the Erasmus MC and the effects and implementation were evaluated using a pre-post study design. 💡 Night shift-related insomnia, need for recovery, and fatigue decreased significantly after the intervention! 🤝 Facilitators for future successful implementation are mostly at the organizational level, including a positive attitude towards the intervention, clear guidelines regarding intervention elements, appointment of night workers as ambassadors, and suitable conditions in terms of work demands and for the intervention elements. 📜 Want to know more? Be Consistent: Just as we emphasize consistency in patient care, maintaining a regular bedtime routine helps signal to our bodies that it's time to unwind and prepare for restorative sleep. 2. Bedroom Environment: Creating a serene and comfortable sleep environment is paramount.
