As I think about some of the amazing things we are about to undertake (by the way... STAY TUNED!!!) at Unite Us, I found myself reflecting on the journey to get here. We've faced a lot of "no" and "we don't do things that way," but also many breakthroughs.
I'm incredibly grateful for the work we've done. We've had the privilege of helping countless individuals by addressing social determinants of health (SDoH) and improving health outcomes in our communities. The impact is truly rewarding, and there's still so much more to do.
I'm proud to be part of this wave of change, striving to create a healthier, more equitable future for all. Thank you to everyone who has been part of this journey, especially the communities doing the hard work, the partners pushing the system, and the innovative thinkers building the future.
- Building the country's first statewide care coordination and community organization medicaid reimbursement model AND proving the value of this work through better health outcomes and lowered total costs of care (North Carolina Healthy Opportunities Pilot)
- Driving investments into organizations to coordinate wrap around supports and interventions to get 20 unhoused families into stable housing and helping 175 clients stay housed (United Way of Southwest Missouri & Southeast Kansas)
- Improving health outcomes for moms and babies and reducing post-partum readmission rates by almost 80%! (First 1,000 Days Suncoast & Sarasota Memorial Health Care System)
- Financially stabilizing rural hospitals with a focus on whole-person care and through innovative Medicaid programming via the ToRCH model (Missouri Department of Social Services)
None of this is possible without amazing partners, co-workers, colleagues, and visionaries.
