I've been a weird mixture of cranky and self-conscious ever since last Friday, when I began a year long treatment of Invisalign to straighten my teeth. Cranky ➡️ because it makes your teeth ache for the first few days. Self-conscious ➡️ because I'm middle-aged now and I feel like a kid wearing braces... But at the same time... I feel like a senior citizen who needs to take out his dentures a few times a day. CLICK-CLICK. UCH. ________ I've got 51 more weeks left of this.... And then, I'll have nice, straight, white teeth again. I can already anticipate the effect of having an open-mouth, uninhibited, smile on my disposition, and I'm really looking forward to it. But it got me thinking: __________ If I just applied a small amount of daily discomfort and self-consciousness... Having wisdom teeth removed can be like a rite of passage for young adults, even though some people don't even get wisdom teeth and others have enough room to fit the full set. But how do wisdom teeth affect orthodontic treatment? Wisdom teeth have minimal impact on dental crowding, so they usually won't need to be removed just to improve the alignment of the other teeth. Luckily, if they do need to be extracted (whether because they're impacted and endangering the roots of neighboring teeth or there isn't enough room for them to come in), that doesn't have to interrupt orthodontic treatment. Many teenagers get braces well before the age when wisdom teeth removal would be considered, and there's a persistent myth that wisdom teeth can cause a post-braces smile to become crooked again. Our teeth do naturally shift as we age, but that's not because of wisdom teeth. Many times I have had a patient tell me they have done orthodontic treatment 2 or 3 times, and their teeth just can’t stay put. 🚩BIG RED FLAG 🚩in my myo book! I can’t tell you how many times in this situation my patient has a tongue-tie, tongue thrust, and most of the time they have both. Why are we continuing to put a patient in orthodontic treatment again, only for it to fail?! Let’s get it right the first time. Some discomfort or mild pain is normal, especially when you first get your braces put on or adjusted. An easy way to get relief is a saltwater rinse – gargle with the solution, letting it sit on sore areas before spitting it out. Also, keep your braces wax on hand at all times! These will really help the inside of your cheeks and lips while you are getting use to the new appliances in your mouth. Harmful childhood habits like thumb sucking past toddlerhood, nail-biting, tongue thrusting, and mouth breathing can all contribute to orthodontic problems such as a narrow upper arch, an underdeveloped lower jaw, an open bite, or dental crowding. 😯 These can all make it more difficult to speak clearly or chew and swallow effectively, and they can be corrected with braces but parents might be able to head them off early by discouraging the habits causing them. Habitual mouth breathing in childhood can cause a variety of problems, both in the short term and over time. When the mouth is closed, the tongue provides the right pressure for a child's dental arches and facial bones to develop well. Mouth breathing removes that important support structure and leads to narrow arches (along with flat features, drooping eyes, and a small chin). That will make dental crowding much more likely. Parents should try to encourage nose breathing whenever possible. 👃 Past a certain age, as adult teeth are developing in a child's jaw, frequent and vigorous thumbsucking or pacifier use begin to have harmful effects on how those adult teeth will grow in, and they can even change the shape of the dental arch.
